摘要
目的探讨肾黏液样小管状和梭形细胞癌(MTSCC-K)的影像学特征及病理特点。方法回顾性分析2015年9月至2021年11月6例经手术病理证实的6例MTSCC患者的影像学资料,观察肿瘤的部位、大小、形态、边界、CT密度(有无坏死囊变、出血、钙化)、MRI信号、增强扫描特点、周围侵犯及远处转移等情况,并作文献复习。结果6例术前均行CT检查,位于右肾4例,左肾2例;最大径2.0~7.6 cm;6例边界均清楚;5例为类圆形,1例为楔形;3例有假包膜;CT平扫3例呈均匀等或低密度,3例密度不均匀(1例有出血,1例环形钙化,1例有出血、囊变及钙化),2例同时行MRI检查,T1WI序列等低或低信号为主,T2WI序列等高或高信号为主,1例见环形低信号,DWI呈高信号,ADC为低信号,增强:呈缓慢渐进性轻中度强化和平台强化相结合。结论MTSCC的CT和MRI表现具有一定的特征性,且与病理密切相关,充分认识这些特征有助于MTSCC的术前诊断。
Objective To investigate the imaging and pathology characteristics of mucinous tubular and spindle cell carcinoma of the kidney(MTSCC-K).Methods The imagine data of 6 patients with MTSCC-k were retrospectively analyzed from September 2015 to November 2021,which were confirmed by postoperative histopathology.The location,size,morphology,boundary,CT density(with or without necrotic cystic change,hemorrhage and calcification),MRI signal,enhanced scanning characteristics,peripheral invasion and distant metastasis of the tumor were observed,and the literature was reviewed.Results 6 patients underwent CT scanning,4 cases were in right kidney and 2 cases were in left kidney.The maximum diameter of tumor ranged from 2.0 to 7.6 cm.All 6 cases had clear boundaries.5 cases were quasi-circular and 1 case was wedge-shaped.3 cases had false capsule.Unenhanced CT images showed equal or low density in 3 cases,3 cases had uneven density(1 patients had bleeding,1 cases had annular calcification,1 cases had bleeding,cystic degeneration and calcification).2 cases also underwent MRI scanning,T1WI was dominated by equal,low or mixed low signal,and T2WI was dominated by medium,high or mixed high signal,1case had annular low signal,high DWI signal,but low ADC value.Enhancement:progressive mild to moderate enhancement.Conclusion MTSCC has certain characteristic CT and MRI findings,closely related to pathology.Fully recognize of these characteristics is helpful for preoperative diagnosis of MTSCC.
