摘要
目的探讨ETButton-AC系统与锁骨钩钢板在治疗TossyⅢ型肩锁关节脱位的应用效果。方法对2022年1月至2022年6月在南京医科大学第二附属医院骨科需要行手术治疗的49例TossyⅢ型肩锁关节脱位患者的临床疗效进行对比分析,其中26例使用ETButton-AC系统治疗,23例使用AO锁骨钩钢板治疗。观察两组患者的术中出血量、手术时间、住院天数、恢复日常活动所需时间、肩关节ASES评分及术后并发症发生率。结果ETButton-AC组的手术时间略大于锁骨钩钢板组,住院时长少于锁骨钩钢板组,但差异均无统计学意义(P>0.05);ETButton-AC组的术中出血量、恢复日常活动所需时间明显少于锁骨钩钢板组,差异有统计学意义(P<0.05);ETButton-AC组术后1、3个月的ASES评分(包括VAS疼痛评分与平均累计日常活动评分)均明显优于锁骨钩钢板组,差异有统计学意义(P<0.05);ETButton-AC组术后总的并发症发生率相对较低,但差异无统计学意义(P>0.05)。结论相比于传统意义的锁骨钩钢板治疗TossyⅢ型肩锁关节脱位,ETButton-AC系统可进一步减少术中出血量,改善术后患者肩关节疼痛情况和日常活动情况,且术后活动受限等并发症发生率相对较低。
Objective To compare the effect of the ETButton-AC system and the clavicular hook plate in the treatment of Tossy typeⅢacromioclavicular joint dislocation.Methods From January 2022 to June 2022,49 patients with Tossy typeⅢacromioclavicular joint dislocation in the Department of Orthopedics of the Second Affiliated Hospital of Nanjing Medical University were compared and analyzed.In this study,26 cases were treated with ETButton-AC system and 23 cases with AO clavicular hook plate.The amount of intraoperative blood loss,the time of operation,the days of hospitalization,the time of resuming daily activity,the ASES score of shoulder joint and the occurrence of postoperative complications were observed.Results The operation time of the ETButton-AC group was slightly longer than that of the clavicular hook plate group and the length of hospital stay was shorter than that of the clavicle hook plate group,but the differences was not statistically significant(P>0.05);the amount of intraoperative blood loss and the time needed to resume daily activities in the ETButton-AC group were significantly less than those in the clavicular hook plate group and the difference was statistically significant(P<0.05).The ASES scores(including VAS pain score and average cumulative daily activity score)in the ETButton-AC group at 1 month and 3 months after operation were significantly better than those in the clavicular hook plate group,and the difference was statistically significant(P<0.05).The total incidence of postoperative complications in the ETButton-AC group was relatively low,but the difference was not statistically significant(P>0.05).Conclusion Compared with the traditional clavicular hook plate in the treatment of acromioclavicular joint dislocation,ETButton-AC system can further reduce intraoperative blood loss and improve postoperative shoulder pain and activity.The incidence of postoperative complications such as limited movement is relatively low.
