摘要
目的观察中医定向透药联合通络祛痛膏外敷治疗盘源性腰痛(DLBP)的临床疗效。方法选择2020年8月至2021年11月就诊DLBP患者62例,按照随机数字表法分为2组,治疗组32例予中医定向透药联合通络祛痛膏外敷治疗,对照组30例单纯予中医定向透药治疗。2组均治疗10天后统计疗效,比较2组治疗前后疼痛视觉模拟评分法(VAS)评分变化情况,比较2组治疗后腰椎功能改良Macnab疗效评定标准评价分级情况。结果治疗组总有效率90.63%(29/32),对照组总有效率80.00%(24/30),治疗组总有效率高于对照组(P<0.05)。与本组治疗前比较,2组治疗后疼痛VAS评分均降低(P<0.05),且治疗组治疗后疼痛VAS评分低于对照组(P<0.05)。治疗组优良率81.25%(26/32),对照组优良率66.67%(20/30),治疗组腰椎功能改良Macnab疗效评定标准评价分级优良率高于对照组(P<0.05)。结论中医定向透药联合通络祛痛膏治疗DLBP临床疗效确切,可有效缓解患者疼痛症状,提高腰椎功能,既能发挥中医定向透药的药物穿透优势,又能发挥外敷药物的连续持久作用,且操作简单,安全方便。
Objective To observe the clinical efficacy of directional penetration of traditional Chinese medicine(TCM)combined with external application of Tongluo Qutong Plaster for discogenic low back pain(DLBP).Methods A total of 62 DLBP patients treated in our hospital from August 2020 to November 2021 were randomly assigned into the treatment group(n=32)and the control group(n=30).All patients were managed by directional penetration of TCM,and those in the treatment group were additionally treated with external application of Tongluo Qutong Plaster.Treated for 10 days,the aim was to compare visual analogue scale(VAS)for pain,modified MacNab criteria.The curative effect was assessed.Results The total effective rate of the treatment group was higher than that of the control group(90.63%[29/32]vs 80.01%[24/30],P<0.05).VAS scores of pain in the treatment group were significantly lower than those in the control group(P<0.05),and the decrease was more pronounced in the treatment group than in the control group(P<0.05).The excellent rates in the treatment group and the control group were 81.26%(26/32)and 66.67%(20/30),respectively.The excellent rates indicated by modified MacNab grading criteria for lumbar function in the treatment group were significantly higher than those of the control group(P<0.05).Conclusion For patients with DLBP,the directional penetration of TCM combined with external application of Tongluo Qutong Plaster has a curative effect.It can effectively relieve the pain symptoms of patients and improve the function of lumbar spine.It can not only give full play to directional penetrating advantages of TCM,but also play a lasting role of external application drugs.It is simple,safe and convenient to operate.
