二维超声斑点追踪成像对早产新生儿呼吸窘迫综合征左室心肌力学短期改变的评估价值

摘要 目的:分析二维超声斑点追踪成像(2D-STE)对早产新生儿呼吸窘迫综合征(NRDS)左室心肌力学短期改变的评估价值。方法:选取2019年7月—2022年6月广西钦州市妇幼保健院新生儿科收治的120例早产NRDS患儿为研究对象,运用常规超声心动图及2D-STE评估分析早产NRDS患儿治疗前及治疗后2个不同时点(治疗后12、24 h)的左室功能相关参数。结果:120例患儿治疗前及治疗后12、24 h的左室舒张末期容积、左室收缩末期容积、左室射血分数比较,差异无统计学意义(P>0.05)。治疗前与治疗后12 h的左心室整体纵向应变(GLS)、快速射血期峰值应变率(SRs)、快速充盈期峰值应变率(SRd)比较,差异有统计学意义(P<0.05);治疗前与治疗后24 h的GLS、左心室游离壁快速射血期峰值运动速度(Vs)、SRs、快速充盈期峰值运动速度(Vd)和SRd比较,差异有统计学意义(P<0.05);治疗后12 h与24 h的GLS、SRs比较,差异有统计学意义(P<0.05)。结论:2D-STE可实现对早产NRDS患儿治疗前后左室心肌力学短期变化的定量评估,较常规超声心动图可更早地反映患儿心肌功能异常,为指导临床诊疗、判断预后提供依据。 Objective:To analyze the diagnostic value of two-dimensional ultrasound speckle tracking imaging(2D-STE)for short-term changes in left ventricular myocardial mechanics in preterm neonates with respiratory distress syndrome(NRDS).Methods:120 children with preterm NRDS admitted to the neonatology department of Qinzhou Maternal and Child Healthcare Hospital,Guangxi,China from July 2019 to June 2022 were selected as the study subjects.Conventional echocardiography and 2D-STE assessment were applied to analyze the left ventricular function-related parameters before and 12 and 24 hours after treatment.Results:left ventricular end-diastolic volume,left ventricular end-systolic volume,and left ventricular ejection fraction between 120 children before the treatment and 12 and 24 h after treatment showed no statistically significant differences(P>0.05).Comparison of the eft ventricular overall longitudinal strain(GLS),peak strain rate during rapid ejection(SRs),and peak strain rate during rapid filling(SRd)of the left ventricle before and 12 h after treatment showed statistically significant differ⁃ences(P<0.05);GLS,peak velocity of rapid ejection motion of the free wall of the left ventricle(Vs),SRs,peak velocity of rapid filling motion before and 24 h after treatment were compared,and the differences were statistically significant(P<0.05);GLS and SRs at 12 h after treatment were compared with those at 24 h after treatment.Conclusion:2D-STE can realize quantitative assess⁃ment of short-term changes in left ventricular myocardial mechanics before and after treatment in children with preterm NRDS,and reflect abnormalities in myocardial function of children earlier than conventional echocardiography.It provides a basis clini⁃cal diagnosis and prognosis.
