摘要
目的探索单孔+1吲哚菁绿荧光示踪全腹腔镜胃癌根治术可行性、安全性及技术经验。方法回顾性分析2020年10月至2022年6月在四川省人民医院胃肠外科完成的9例单孔+1吲哚菁绿荧光示踪全腹腔胃癌根治术患者的临床资料,复习手术视频,观察术中和术后并发症发生情况,并总结优化手术流程。结果共纳入9例患者,其中远端胃癌根治术7例,保留幽门胃癌根治术2例,体质指数(23.06±3.92)kg/m^(2),淋巴结检出数目(26.68±6.42)枚。术后早期并发症主要为保留幽门胃癌根治术后发生胃排空延迟2例,均保守治疗后好转。结论单孔+1全腹腔镜胃癌根治术近期疗效良好,安全可靠,腹壁美观度更高,术后疼痛轻,有利于术后恢复,吲哚菁绿荧光示踪可进一步保障肿瘤学安全。
Objective To explore the feasibility,safety and technical experience of SILS+1 indocyanine green fluorescence tracer in total laparoscopic gastric cancer radical operation.Methods From October 2020 to June 2022,clinical data of 9 patients were retrospectively analyzed in the department of Gastrointestinal Surgery of Sichuan People's Hospital.Surgical videos were reviewed,intraoperative and postoperative complications were observed,and surgical procedures were summarized and optimized.Results Total 9 patients were included,including 7 TLDG and 2 TLPPG with a BMI of(23.06±3.92)kg/m^(2).Lymph nodes detected was(26.68±6.42).The early postoperative complication was delayed gastric emptying after pyloric-sparing gastrectomy in 2 cases,all of which were improved after conservative treatment.Conclusion The short-term efficacy of SILS+1 laparoscopic surgery for gastric cancer is safe and feasible,with higher abdominal wall aesthetics and less postoperative pain,which is conducive to postoperative recovery.Indocyanine green fluorescence tracer could be further guarantee oncology safety.
