摘要
目的探究P波时限对阵发性房颤患者消融术后复发的影响,并对其预测价值进行评估。方法依照纳入及排除标准,选择本院2020年5月至2022年2月首次使用射频消频术进行治疗的84例阵发性房颤患者,并依照术后随访判断是否出现阵发性房颤复发症状将患者归入复发组(n=16)及未复发组(n=68),所有患者术前和术后3个月时行12导联心电图检查,测量Ⅱ及V1导联的P波时限记录数据后通过SPSS25.0软件分析。结果单因素显示复发组合并高血压的概率明显大于未复发组,有无高血压、心率、Ⅱ及V1导联Pmax、Pd值显著大于未复发组(P<0.05),多因素显示Ⅱ导联和V1导联Pd值≥46ms,为阵发性房颤患者出现术后复发的危险因子(P<0.05)。ROC曲线显示Ⅱ导联Pd值≥46ms的灵敏度为64.26%,V1导联Pd≥46ms的灵敏度为65.35%,Ⅱ导联Pd值预测阵发性房颤术后复发的ROC曲线下面积为0.882,V1导联Pd值预测阵发性房颤术后复发的ROC曲线下面积为0.865。结论Ⅱ及V1导联Pd值可以作为预测阵发性房颤术后复发的有效预测因素,为临床早期预防及诊断阵发性房颤术后复发提供了依据。
Objective To explore the influence of P wave duration on recurrence after ablation in patients with paroxysmal atrial fibrillation and evaluate its predictive value.Methods According to the inclusion and exclusion criteria,84 patients with paroxysmal atrial fibrillation who were treated by radiofrequency ablation for the first time in our hospital from May 2020 to February 2022 were selected.According to the postoperative follow-up,the patients were divided into recurrent group(n=16)and non-recurrent group(n=68).All patients underwent 12-lead ECG examination before operation and 3 months after operation,and the P-wave duration recorded data in leadsⅡand V_1 were measured and analyzed by SPSS 25.0 software.Results Univariate analysis showed that the probability of recurrent combination complicated with hypertension was significantly higher than that of non-recurrent group,and the presence or absence of hypertension,heart rate,Pmax and Pd values in leadsⅡand V_1 were significantly higher than those in non-recurrent group(P0.05).Multivariate analysis showed that Pd values in leadsⅡand V_1 were≥46ms,which was a risk factor for postoperative recurrence in patients with paroxysmal atrial fibrillation(P0.05).The sensitivity of ROC curve for Pd value≥46ms in leadⅡwas 64.26%,and that of Pd value≥46ms in lead V_1was65.35%.The area under ROC curve for predicting recurrence of paroxysmal atrial fibrillation in leadⅡwas 0.882,and that under ROC curve for predicting recurrence of paroxysmal atrial fibrillation in lead V_1 was 0.865.Conclusions The Pd value ofⅡand V_1 leads can be used as effective predictors of postoperative recurrence of paroxysmal atrial fibrillation,providing a basis for early clinical prevention and diagnosis of postoperative recurrence of paroxysmal atrial fibrillation.
