摘要
目的:探讨多学科协作护理对高龄胆总管切开手术患者胃肠功能恢复及并发症发生情况的影响。方法:选取2020年5月—2022年5月南京市第一医院接诊并拟行胆总管切开手术的高龄患者98例作为研究对象,采用随机数字表法分为观察组与对照组,各49例。对照组予以常规护理,观察组在对照组基础上予以多学科协作护理。比较两组胃肠激素水平、胃肠功能及并发症发生情况。结果:出院前1天,两组胃泌素、胃动素水平均低于手术前,但观察组高于对照组,差异有统计学意义(P<0.05)。观察组术后肠鸣音恢复时间、首次进食时间、首次肛门排气时间、首次排便时间、下床活动时间均短于对照组,差异有统计学意义(P<0.05)。观察组并发症总发生率低于对照组,差异有统计学意义(P=0.031)。结论:多学科协作护理有助于调节高龄胆总管切开手术患者术后胃肠激素水平,促进胃肠功能恢复,预防并发症。
Objective:To explore the influence of multidisciplinary cooperative nursing on gastrointestinal function recovery and complications in elderly patients undergoing choledochotomy.Methods:A total of 98 elderly patients who were admitted to Nanjing First Hospital and planned to undergo choledochotomy from May 2020 to May 2022 were selected as the study subjects.They were divided into control group and observation group according to random number table method,with 49 cases in each group.The control group was given routine nursing,and the observation group was given multidisciplinary cooperative nursing on the basis of the control group.The levels of gastrointestinal hormones,gastrointestinal function and complications were compared between the two groups.Results:On 1 day before discharge,the levels of gastrin and motilin in two groups were lower than those before surgery,but the observation group was higher than the control group,and the difference was statistically significant(P<0.05).The postoperative bowel sound recovery time,first eating time,first anal exhaust time,first defecation time,and ambulation time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.031).Conclusion:Multidisciplinary cooperative nursing is helpful to regulate the levels of postoperative gastrointestinal hormones in elderly patients undergoing choledochotomy,promote the recovery of gastrointestinal function and prevent complications.
