摘要
目的 探讨动态增强磁共振成像(Dynamic enhanced magnetic resonance imaging;,DCE-MRI)联合多层螺旋CT在胃癌患者中的诊断价值。方法 选取我院就诊的疑似早期胃癌患者90例作为观察对象,随机分为观察组和对照组各45例,观察组使用DCE-MRI联合多层螺旋CT进行检测,对照组使用多层螺旋CT检测。比较两组平扫CT值、增强峰值、强化增值;比较两组平均通过时间(MTT)、表面通透性(PS)、血容量(BV)、血流量(BF)数值;比较两组血浆空间容积分数(V_p)、血管外细胞外容积分数(V_e)、对比剂从血浆渗漏至血管外细胞外间隙体积转运常数(K~(trans))、对比剂从血管外细胞外间隙返回至血浆空间速率常数(K_(ep))。结果 观察组平扫CT值高于对照组,差异有统计学意义(P<0.05),两组增强峰值、强化增值差异无统计学意义(P>0.05)。两组MTT、PS、BV、BF差异无统计学意义(P>0.05)。两组V_e、V_p、K~(trans)、K_(ep)数值差异无统计学意义(P>0.05)。关于灵敏度与阴性预测值,DCE-MRI和多层螺旋CT联合检测对诊断胃癌的价值,高于单独检测,差异有统计学意义(P<0.05)。结论 DCE-MRI联合多层螺旋CT检测灵敏度、阴性预测值均高于单独检测,对胃癌患者具有明显诊断价值。
Objective To explore the diagnostic value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)combined with multi-slice spiral CT in patients with gastric cancer.Methods A total of 90 patients with suspected early gastric cancer who were treated in our hospital from April 2019 to May 2020 were selected as the observation objects,with 45 patients in each group.The observation group was detected by DCE-MRI combined with multi-slice spiral CT technology,and the control group was detected by multi-slice spiral CT technology.Plain CT value,enhancement peak value and enhancement increment value were compared between the two groups.Mean transit time(MTT),surface permeability(PS),blood volume(BV)and blood flow(BF)were compared between the two groups.Plasma space volume fraction(V p),extracellular volume fraction(V e),volume transport constant of contrast agent leakage from plasma to extracellular space(K trans)and space rate constant of contrast agent return from extracellular space to plasma(K ep)were compared between the two groups.Results The plain CT value in the observation group was higher than that in the control group(P<0.05),and there was no statistical significance in the enhancement peak value and enhancement increment value between the two groups(P>0.05).There were no significant differences in MTT,PS,BV and BF between the two groups(P>0.05).There were no significant differences in V e,V p,K trans and K ep between the two groups(P>0.05).Regarding the sensitivity and negative predictive value,the value of combined detection of DCE-MRI and multi-slice spiral CT in the diagnosis of gastric cancer was higher than that of single detection(P<0.05).Conclusion The detection sensitivity and negative predictive value of DCE-MRI combined with multi-slice spiral CT technology are higher than those of single detection,which has obvious diagnostic value for gastric cancer patients.
