摘要
目的:比较不同年龄阶段女性附件扭转(AT)的超声特征和临床表现。方法:回顾性分析2014年1月—2023年6月在宜昌市中心人民医院经手术确诊为AT的105例女性患者临床资料。根据患者的就诊年龄分为三组:未成年组(n=21),育龄期组(n=58)和已绝经组(n=26)。结果:105例AT患者平均年龄为35.17±18.21岁,超声明确诊断90例,超声诊断符合率为85.71%。已绝经患者的附件肿物最大直径平均值(110.31±35.30 mm)较育龄期组(90.50±34.00 mm)和未成年组(76.20±33.00 mm)明显增大(P<0.05)。未成年组发生恶心/呕吐及盆腔积液更常见(均P<0.05)。在手术方式上,未成年组患者行肿物剔除+扭转复位术更多(P<0.001),已绝经组行患侧附件切除术更多(P<0.001)。未成年组和育龄期组患者中最常见的病理类型均为卵巢成熟性畸胎瘤,绝经期组最常见的病理类型为卵巢单纯性囊肿。结论:不同年龄阶段AT患者临床表现、超声特征、手术方式及病理类型均有异同,熟悉不同年龄阶段AT患者的相关特征对快速诊断和准确治疗至关重要。
Objective:To compare the ultrasonic features and clinical manifestations of adnexal torsion(AT)in female patients at different age.Methods:A total of 105 female patients diagnosed with AT after surgery in Yichang Central People's Hospital from January 2014 to June 2023 were retrospectively analyzed.Clinical data of all patients were collected and divided into three groups according to the age of treatment:the juvenile group(n=21),the childbearing group(n=58)and the postmenopausal group(n=26).Results:The mean age was 35.17±18.21 years old.There were 90 cases diagnosed by ultrasound,with the diagnosis coincidence rate of 85.71%.The mean maximum diameter of appendage mass in the postmenopausal group(110.31±35.30 mm)was significantly higher than that in the childbearing group(90.50±34.00 mm)and juvenile group(76.20±33.00 mm)(P<0.05).Nausea/vomiting and pelvic effusion were more common in the juvenile group(both P<0.05).More patients underwent tumor removal combined with torsion reduction in the juvenile group(P<0.001).More patients underwent adnexectomy in the postmenopausal group(P<0.001).The most common pathological type among patients in the juvenile and childbearing groups were ovarian mature teratoma.Whereas ovarian simple cyst was most common in the postmenopausal group.Conclusion:There are differences in clinical manifestations,ultrasound features,surgical methods and pathological types of AT patients at different ages.It is important to grasp the relevant characteristics of AT patients at different ages for rapid diagnosis and accurate treatment.
