摘要
目的探讨成纤维细胞生长因子21(FGF21)表达水平与溃疡性结肠炎(UC)内窥镜下活动度的关系。方法选取我院2021年3月~2023年3月收治的106例UC患者,内窥镜结果显示均为活动期,按照病情严重程度分为轻中度活动组(n=83)和重度活动组(n=23)。对比两组患者的一般资料和相关指标,分析FGF21表达水平与患者其他指标的相关性,采用Logistic回归分析影响重度UC的因素,分析FGF21表达水平对重度UC的预测价值。结果重度活动组的UCEIS评分、ESR、CRP、PLT、WBC、Treg、Th17、Th17/Treg及FGF21表达水平均高于轻中度活动组(P<0.05),两组患者的性别、年龄、体重指数、病程、病变程度、RDW、MCV、Hb、Alb对比差异无统计学意义(P>0.05);Pearson相关性分析结果显示,FGF21表达水平与UCEIS评分、ESR、CRP、PLT、WBC、Th17、Th17/Treg呈正相关(P<0.05),与Treg呈负相关(P<0.05);Logistic回归分析显示,WBC、CRP、PLT、Th17/Treg、ESR、FGF21表达水平升高为影响重度UC的独立危险因素(P<0.05);ROC曲线分析显示,FGF21表达水平对重度UC的预测灵敏度为91.42%,特异度为89.53%,相较于其他因素的预测价值较高。结论FGF21表达水平可显示UC的活动度,评估患者的病情,且具有较高的预测价值。
Objective To investigate the relationship between the expression level of fibroblast growth factor 21(FGF21)and the endoscopic activity of ulcerative colitis(UC).Methods 106 patients with UC enrolled in our hospital from March 2021 to March 2023 were selected.According to the endoscopic results,they were all active patients,and were divided into mild to moderate activity group(n=83)and severe activity group(n=23)according to the severity of the disease.The general data and relevant indicators of the two groups of patients were compared,and the correlation between the expression level of FGF21 and other indicators of the patients was analyzed.The factors affecting severe UC were analyzed by Logistic regression,and the predictive value of the expression level of FGF21 on severe UC was analyzed.Results The UCEIS score,ESR,CRP,PLT,WBC,Treg,Th17,Th17/Treg and FGF21 expression level in the severe activity group were higher than those in the mild to moderate activity group(P<0.05).There were no significant difference in gender,age,body mass index,disease course,lesion degree,RDW,MCV,Hb and Alb between the two groups(P>0.05).Pearson correlation analysis showed that the expression level of FGF21 was positively correlated with UCEIS score,ESR,CRP,PLT,WBC,Th17,Th17/Treg(P<0.05),and negatively correlated with Treg(P<0.05).Logistic regression analysis showed that WBC,CRP,PLT,Th17/Treg,ESR and FGF21 expression level were independent risk factors for severe UC(P<0.05).ROC curve analysis showed that the sensitivity and specificity of FGF21 expression level in predicting severe UC were 91.42%and 89.53%,which were relatively high compared with other factors.Conclusion The expression level of FGF21 can show the activity of UC,evaluate the disease of patients,and has high predictive value.
