摘要
目的:探讨对妊娠糖尿病(GDM)患者予门冬胰岛素联合地特胰岛素治疗的效果。方法:选取2021年2月—2023年2月在广州市妇女儿童医疗中心诊治的100例GDM患者,通过奇偶法将其分成对照组和治疗组,各50例。对照组采取门冬胰岛素治疗,治疗组采取门冬胰岛素+地特胰岛素治疗。比较两组临床效果,空腹血糖(FBG)、餐后2 h血糖(2 hPBG)及二者均达标时间,糖化血红蛋白(HbA1C),胰岛素抵抗指数(HOMA-IR),胰岛β细胞功能指数(HOMA-β),不良妊娠结局。结果:治疗组治疗总有效率高于对照组,差异有统计学意义(P=0.045);治疗后,治疗组Omentin-1、HOMA-β水平高于对照组,差异有统计学意义(P<0.001);治疗组FBG、2 hPBG、二者均达标时间早于对照组,治疗后FBG、2 hPBG、HbA1C、HOMA-IR水平低于对照组,差异有统计学意义(P<0.05);治疗后,治疗组不良妊娠结局发生率低于对照组,差异有统计学意义(P=0.037)。结论:门冬胰岛素联合地特胰岛素治疗GDM的效果显著,可缩短血糖达标时间,降低血糖水平,对改善母婴结局存在重要意义。
Objective:To investigate the effect of insulin aspartate combined with insulin detemir in patients with gestational dia⁃betes mellitus(GDM).Methods:A total of one hundred GDM patients diagnosed and treated in Guangzhou Women and Children Medical Center from February 2021 to February 2023 were selected and divided into control group and treatment group by oddeven method,with fifty cases in each group.The control group was treated with insulin aspartate,and the treatment group was treated with insulin aspartate+insulin detemir.The clinical effects of the two groups were compared,including fasting blood glu⁃cose(FBG),2 h postprandial blood glucose(2 hPBG)and the time when both reached the standard,glycated hemoglobin(HbA1C),insulin resistance index(HOMA-IR),islet beta cell function index(HOMA-β)and adverse pregnancy outcome.Results:The total effective rate of treatment group was higher than that of control group,the difference was statistically significant(P=0.045);After treatment,the Omentin-1,HOMA-βlevel in the treatment group was higher than that in the control group,and the difference was statistically significant(P<0.001);The standard time of FBG,2 hPBG in the treatment group was earlier than that in the control group,and the level of FBG,2 hPBG,HbA1C,HOMA-IR after treatment was lower than that in the control group,with statistical significance(P<0.05);The incidence of adverse pregnancy outcome in treatment group was lower than that in control group,and the difference was statistically significant(P=0.037).Conclusion:Insulin aspartate combined with insulin detemir has a significant effect in the treatment of GDM,which can shorten the time of blood glucose compliance and reduce blood glucose level,which is of great significance for improving maternal and infant outcomes.
