摘要
目的对面部偏斜患者的颌面部结构进行锥形束CT三维扫描成像并测量,分析面部偏斜患者下颌骨的结构特征以及空间位置。方法选取面部偏斜的患者61例为研究组(男7例,女54例),年龄(23.97±5.80)岁,选取15例个别正常[牙合]患者为对照组(男2例,女13例),年龄(22.47±5.49)岁,进行病例对照研究。对所选取研究对象进行颅面部锥形束CT成像,利用三维测量软件(Invivo5)的3D分析功能描绘测量相关解剖标志点,比较两组患者下颌骨形态与位置差异。结果①研究组偏斜侧下颌升支高度(54.78±6.30)mm与偏斜对侧(56.22±5.25)mm(P=0.004),偏斜侧下颌体总长度(116.15±6.00)mm与偏斜对侧(117.96±6.20)mm(P=0.001),偏斜侧髁突高度(23.34±3.80)mm与偏斜对侧(24.50±3.40)mm(P=0.001),偏斜侧上颌尖牙垂直向位置(24.72±4.35)mm与偏斜对侧(25.29±4.32)mm(P=0.028),偏斜侧上颌第一磨牙垂直向位置(22.60±4.51)mm与偏斜对侧(23.13±4.12)mm(P=0.041),偏斜侧下颌升支内倾角度84.21°±3.16°与偏斜对侧82.87°±3.81°(P=0.015)比较,差异均具有统计学意义,对照组差异均无统计学意义。②研究组偏斜侧与偏斜对侧下颌体长度差异无统计学意义。结论成年面部偏斜患者双侧存在多部位解剖结构的不对称性,偏斜侧的下颌升支高度、髁突高度更小,牙弓平面更高,下颌升支内倾角度更大。
Objective Three-dimensional cone-beam CT imaging was performed to measure the maxillofacial structure of patients with facial deviation.To analyze the structural features and spatial position of mandible in patients with facial deviation.Methods A total of 61 patients(7 males and 54 females)with facial deviation were selected as the study group,aged(23.97±5.80)years;and 15 patients with individual normal occlusion(2 males and 13 females)were selected as the control group,aged(22.47±5.49)years.A case-control study was conducted.Craniofacial cone-beam CT imaging was performed on the selected subjects,and the 3D analysis were fulfilled with software invivo5,which also detected the relevant anatomical landmarks,and the differences in mandible morphology and position between the two groups were compared.Results①In the study group,the height of mandibular ramus on the deviated side was(54.78±6.30)mm and that on the contralateral side was(56.22±5.25)mm(P=0.004).The total length of mandibular body on the deviated side was(116.15±6.00)mm and the total length of mandibular body on the deviated side was(117.96±6.20)mm(P=0.001).The height of condyle on the deviated side was(23.34±3.80)mm and the height of condyle on the deviated side was(24.50±3.40)mm(P=0.001).The vertical position of maxillary cuspid on the deviated side was(24.72±4.35)mm,and the vertical position of maxillary cuspid on the deviated side was(25.29±4.32)mm(P=0.028).The vertical position of the first maxillary molar on the deviated side was(22.60±4.51)mm,and the vertical position of the first maxillary molar on the deviated side was(23.13±4.12)mm(P=0.041).There were significant differences in the angle of mandibular ramus(84.21°±3.16°)and the angle of mandibular ramus(82.87°±3.81°)on the deviated side(P=0.015).All the above comparisons had no significant difference in the control group.②There was no significant difference in mandibular length between the deviated side and the contralateral side.Conclusions In adult patients with facial d
