摘要
目的分析多层螺旋CT检查诊断胃肠道间质瘤的准确性。方法选取80例疑似胃肠道间质瘤患者,均为开封市中心医院2018年1月至2022年12月所收治,术前全部患者均进行多层螺旋CT检查,并将手术病理检查结果当成诊断金标准,分析多层螺旋CT检查的准确性和诊断价值。结果全部80例疑似胃肠道间质瘤患者中,71例患者经手术病理检查结果确诊;多层螺旋CT检查出胃肠道间质瘤患者64例;将手术病理检查结果当成诊断金标准,多层螺旋CT检查的诊断特异度为66.67%(6/9),诊断敏感度为90.14%(64/71),诊断准确性87.50%(70/80),阴性预测值为46.15%(6/13),阳性预测值为95.52%(64/67)。手术病理检查结果显示,全部71例证实为胃肠道间质瘤的患者中,6例患者为良性,10例患者为潜在恶性,55例患者为恶性;多层螺旋CT检查结果显示,4例患者为良性,7例患者为潜在恶性,40例患者为恶性;将手术病理检查结果当成诊断金标准,多层螺旋CT检查的良性诊断符合率66.67%(4/6),恶性诊断符合率为72.73%(40/55)。在多层螺旋CT影像学特征方面,良性胃肠道间质瘤主要表现为病灶呈类圆形,≤5 cm,边界清晰,均匀强化、密度均匀;而潜在恶性、恶性胃肠道间质瘤的主要表现为病灶呈叶状,>5 cm,边界模糊,侵犯周围器官。结论采用多层螺旋CT检查能对胃肠道间质瘤进行准确地诊断和鉴别诊断,胃肠道间质瘤患者具有显著的CT影像学特征,能对其危险程度进行准确判断,为临床治疗方案的制定提供参考。
Objective To analyze the accuracy of multi-slice spiral CT in diagnosing gastrointestinal stromal tumors.Methods A total of 80 patients with suspected gastrointestinal stromal tumors were selected,all of whom were admitted to Kaifeng Central Hospital from January 2021 to December 2022.All patients underwent multi-slice spiral CT examination before 134 Clinical Research,Oct.2023,Vol.31 No.10 surgery,and the results of surgical pathology were used as the diagnostic gold standard to analyze the accuracy and diagnostic value of multi-slice spiral CT examination.Results Among all 80 suspected gastrointestinal stromal tumors,71 patients were diagnosed through surgical and pathological examination;64 patients with gastrointestinal stromal tumors were detected by multislice spiral CT examination.Using the results of surgical pathology as the diagnostic gold standard,the diagnostic specificity of multi-slice spiral CT was 66.67%(6/9),the diagnostic sensitivity was 90.14%(64/71),the diagnostic accuracy was 87.50%(70/80),the negative predictive value was 46.15%(6/13),and the positive predictive value was 95.52%(64/67).The surgical pathological examination results showed that among all 71 patients with confirmed gastrointestinal stromal tumors,6 patients were benign,10 patients were potentially malignant,and 55 patients were malignant;the results of multi-slice spiral CT examination showed that 4 patients were benign,7 patients were potentially malignant,and 40 patients were malignant.Using the results of surgical pathology as the diagnostic gold standard,the accuracy rate of benign diagnosis with multi-slice spiral CT examination was 66.67%(4/6),and the accuracy rate of malignant diagnosis was 72.73%(40/55).In terms of imaging features of multi-slice spiral CT,benign gastrointestinal stromal tumors are mainly characterized by a quasi circular lesion,≤5 cm,clear boundaries,uniform enhancement,and uniform density;The main manifestation of potential malignant and malignant gastrointestinal stromal tumors is that the lesion i
