摘要
目的探究超声引导下连续腹横肌平面阻滞在剖宫产产妇术后镇痛中的应用效果。方法选取2019年3月至2020年5月秀山土家族苗族自治县人民医院收治的128例剖宫产产妇,依据随机数字表法分成对照组与研究组,每组64例,对照组进行常规的静脉自控镇痛,研究组则在常规镇痛基础上实施超声引导下连续腹横肌平面阻滞方法镇痛。观察静脉自控镇痛用药量、术后不同时间点的视觉模拟评分法(VAS)评分、不良反应发生情况。结果研究组产妇术后4 h、术后8 h、术后12 h、术后24 h、术后48 h的常规静脉自控镇痛用药量少于对照组;研究组产妇术后24 h、术后48 h、术后72 h、术后96 h的VAS评分、术后不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论应用超声引导下腹横肌平面阻滞可以有效减少术后常规静脉自控镇痛用药量,减少术后并发症的发生,保障产妇的安全性,有效缓解剖宫产产妇的疼痛,具有比较理想的术后镇痛效果,值得临床应用。
Objective To explore the effect of ultrasound-guided continuous transversus abdominis plane block on analgesia after cesarean section.Methods A total of 128 cases of cesarean section in the People's Hospital of Xiushan Tujia and Miao Autonomous County from March 2019 to May 2020 were selected and divided into a control group and a study group according to the random number table method,with 64 cases in each group.The control group received routine patient-controlled intravenous analgesia,and the study group received ultrasound-guided continuous transversus abdominis plane block on the basis of routine analgesia.The dosage of patient-controlled intravenous analgesia,visual analogue scale(VAS)score at different time points after surgery,and the occurrence of adverse reactions were observed.Results The dose of routine patient-controlled intravenous analgesia in the study group was lower than that in the control group at 4 h,8 h,12 h,24 h,and 48 h after the operation.The VAS scores of the study group at 24 h,48 h,72 h,and 96 h after delivery and the incidence of postoperative adverse reactions were lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusions Ultrasoundguided transverse abdominis plane block can effectively reduce the amount of conventional intravenous patient-controlled analgesia,reduce the incidence of postoperative complications,ensure the safety of parturient women,effectively relieve the pain of parturient women after cesarean section,and relatively ideal postoperative analgesic effect,which is worthy of clinical application.
