摘要
目的:探讨括约肌间瘘管结扎(LIFT)联合黏膜瓣推移(ERAF)术治疗复杂性肛瘘的临床疗效。方法:选取2020年7月至2022年1月,本院收治的60例复杂性肛瘘患者,随机分为治疗组和对照组各30例。治疗组采用LIFT术联合ERAF术治疗,对照组仅采用LIFT术治疗。比较2组术后疼痛、肛门下坠感、尿潴留、创面感染和肛门功能等。结果:对照组疼痛评分、肛门下坠感评分、尿潴留及创面感染情况均优于治疗组(P<0.05)。治疗组手术时间、术中出血量、住院时间及住院费用均高于对照组(P<0.05)。2组显效率、术后1、3和6个月肛门静息压、肛门最大收缩压及Wexner肛门失禁评分比较,差异无统计学意义(P>0.05)。结论:LIFT术联合ERAF术治疗复杂性肛瘘安全有效,较单纯LIFT术,联合术式不能提高成功率,对瘘管完全成形的复杂性肛瘘可仅行LIFT术治疗。
Objective To investigate the clinical effect of ligation of the intersphincteric fistula tract(LIFT)combined with endorectal advancement flap(ERAF)in the treatment of complex anal fistula(CAF).Methods The 60 patients with CAF admitted in authors hospital(2020-07-2022-01)were ran-domly divided into treatment group(n=30,treated with LIFT combined with ERAF)and control group(n=30,treated with only LIFT),then postoperative pain,anal tenesmus sensation,urinary retention,wound infection and anal function were compared between the two groups.ResuitsThe pain score,anal tenesmus sensation score,urinary retention and wound infection status in control group were better than those in the treatment group(P<0.05).The surgical time,intraoperative bleeding,hospitalization time,and hospitalization expenses in treatment group were higher than those in the control group(P<0.05).There was no statistically significant difference between the two groups in terms of significant effi-ciency,anal resting pressure,maximum anal systolic pressure,and Wexner anal incontinence score at l,3,and 6 months after surgery(P>0.05).Conclusion The combination of LIFT and ERAF is safe and effective in the treatment of CAF.Compared with only LIFT,it can't improve curative effect,the CAF with completely formed fistula could be treated by LIFT procedure without additional operation.
