摘要
目的:评估硒制剂联合中药治疗桥本氏甲状腺炎的临床疗效。方法:检索2012年1月—2022年7月中国知网、万方数据知识服务平台、维普中文期刊服务平台、中国生物医学文献数据库采用硒制剂联合中药(饮片或中成药)治疗桥本氏甲状腺炎的随机对照试验(randomized controlled trial,RCT)文献。根据纳入标准对所有文献进行筛选、质量评价及数据提取,然后运用RevMan 5.4软件进行数据统计分析。结果:纳入9篇RCT文献进行分析,共549例患者,对照组273例,治疗组276例。与对照组(硒制剂)比较,治疗组(硒制剂联合中药)的临床总有效率明显升高[RR=1.35,95%CI(1.21,1.50),P<0.00001];在改善甲状腺球蛋白抗体(TGAb)、甲状腺过氧化物酶抗体(TPOAb)指标方面,治疗组疗效明显优于对照组[分别为SMD=-1.30,95%CI(-1.99,-0.61),P=0.0002;SMD=-1.23,95%CI(-2.06,-0.40),P<0.01]。结论:硒制剂联合中药治疗桥本氏甲状腺炎较单用硒制剂在提高总有效率及改善甲状腺抗体指标方面更具优势。
Objective:To evaluate the clinical efficacy of selenium preparation combined with traditional Chinese medicine in the treatment of Hashimoto′s thyroiditis.Methods:The randomized controlled trial(RCT)literature on the treatment of Hashimoto′s thyroiditis with selenium preparations combined with traditional Chinese medicine(decoction pieces or Chinese patent medicine)from January 2012 to July 2022 was searched from China National Knowledge Infrastructure(CNKI),Wanfang Data Knowledge Service Platform,VIP China Journal Service Platform and China Biology Medicine disc.According to the inclusion criteria,all articles were screened,evaluated for quality and extracted for data.Then RevMan 5.4 software was used for statistical analysis of data Results:9 RCT articles were included for analysis,with a total of 549 patients,273 in the control group and 276 in the treatment group.Compared with the control group(selenium preparation),the clinical total effective rate of treatment group(selenium preparation combined with traditional Chinese medicine)was significantly increased[RR=1.35,95%CI(1.21,1.50),P<0.00001].In terms of improving thyroglobulin antibody(TGAb)and thyroid peroxidase antibody(TPOAb),the curative effect of the treatment group was significantly better than that of the control group[SMD=-1.30,95%CI(-1.99,-0.61),P=0.0002;SMD=-1.23,95%CI(-2.06,-0.40),P<0.01].Conclusion:Selenium preparation combined with traditional Chinese medicine in the treatment of Hashimoto′s thyroiditis has more advantages in improving the total effective rate and thyroid antibody index than selenium preparation alone.
