摘要
目的探讨熵指数与Ramsay评分在危重症儿童镇静评估中的应用价值。方法选取2018年1月至2020年12月在我院儿童重症监护室的接受机械通气的非神经系统疾病患儿63例,根据病情给予咪达唑仑注射液镇静,监测脑功能熵指数及Ramsay镇静评分。对比无镇静及注射镇静药物后不同Ramsay评分下的熵指数,分析熵指数与Ramsay评分相关性,比较镇静前后患者的生命体征及PH、SPO_(2)、PCO_(2)的变化。结果镇静后,患者心率(HR)、呼吸(RR)水平明显下降,PH值及SPO_(2)较镇静前显著好转,差异有统计学意义(P<0.05),而镇静前后患者平均动脉压(MAP)、PCO_(2)水平比较,差异无统计学意义(P>0.05)。SE、RE值与Ramsay评分呈负相关,其相关系数(r)值为[SE:r值为-0.966,(P=0.000)。RE:r值为-0.961,(P=0.000)]。结论熵指数(SE、RE)与Ramsay评分有良好的相关性,在危重症儿童镇静评估中具有一定的应用价值。
Objectives To investigate the application value of entropy index and Ramsay score in the sedation assessment of critically ill children.Methods A total of 63 children with non-neurological diseases who received mechanical ventilation in the Pediatric Intensive Care Unit of Luoyang Maternity and Child Health Hospital from January 2018 to December 2020 were selected.Midazolam injection was given for sedation according to the disease condition.The brain function entropy index and Ramsay sedation score were monitored.The entropy indexes of different Ramsay scores before and after sedation were compared,and the correlation between entropy index and Ramsay score was analyzed.The changes of vital signs,PH,SPO_(2)and PCO_(2)before and after sedation were compared.Results After sedation,the heart rate(HR)and respiration rate(RR)of the patients decreased significantly,and the PH value and SPO_(2)were significantly improved compared with those before sedation(P<0.05),while there was no significant difference in mean arterial pressure(MAP)and PCO_(2)levels before and after sedation(P>0.05).SE and RE values were negatively correlated with Ramsay score,and the correlation coefficient values(r)were[SE:r=0.966,(P=0.000).RE:r=0.961,(P=0.000)].Conclusions Entropy index(SE,RE)has a good correlation with Ramsay score and has certain application value in sedation assessment of critically ill children.
