摘要
介绍Triangle模型的概念、框架及内涵。对该模型在国内外延续护理中应用现状及范围、分层标准等方面进行综述。提出国内未来关于分层分级延续护理的展望,以期为今后开展分层分级延续护理发展提供参考依据。
Introduce the concept,framework and connotation of Triangle model.To reviews the current status and scope of the application of the model in transitional care at home and abroad,as well as the stratifi cation criteria.To propose the domestic future outlook on hierarchical transitional care,in order to provide a reference basis for the development of hierarchical transitional care in the future.
