摘要
目的 研究探讨奥氮平联合氟伏沙明治疗精神分裂症伴抑郁患者其奥氮平使用剂量、N-去甲基奥氮平浓度、血清泌乳素水平的影响.方法 选取2021年1月~2022年11月本院收治的96例精神分裂症伴抑郁患者为对象,随机分为奥氮平治疗48例(对照组),奥氮平联合氟伏沙明治疗48例(观察组).比较两组患者的临床疗效、奥氮平使用剂量、N-去甲基奥氮平浓度、血清泌乳素水平、治疗安全性等.结果 治疗后试验组总有效率为高于对照组(P<0.05);试验组阳性与阴性症状量表(PANSS)量表中阴性和一般精神病理评分低于对照组(P<0.05);试验组汉密尔顿抑郁量表-17项(HAMD-17)评分低于对照组(P<0.05);试验组奥氮平使用剂量、N-去甲基奥氮平、血清泌乳素均低于对照组(P<0.05);两组不良反应发生率相近(P>0.05).结论 奥氮平联合氟伏沙明治疗能够改善精神分裂症伴抑郁的阴性症状和抑郁症状,奥氮平使用剂量更低,降低N-去甲基奥氮平和血清泌乳素水平,治疗安全性良好.
Objective The purpose of this study was to investigate the effects of Olanzapine combined with Flu-voxamine on the dosage of Olanzapine,N-desmethyl Olanzapine,and serum prolactin levels in schizophrenia patients with depression.Methods The study selected ninety-six patients diagnosed with schizophrenia with depression ad-mitted to the hospital from January 2021 to November 2022 as the research subjects.They were randomly divided into two groups:Olanzapine treatment group(control group,n=48)and Olanzapine combined with Fluvoxamine treatment group(experimental group,n=48).The clinical efficacy,Olanzapine dosage,N-desmethyl Olanzapine concentra-tion,serum prolactin levels,and treatment safety were compared between the two groups.Results The total effective rate of the experimental group was higher than that of the control group after treatment(P<0.05).The negative and general psychopathology scores on the Pasitive and negative syndrome scale(PANSS)in the experimental group were lower than those in the control group(P<0.05).The Hamilton Depression Scale-17(HAMD-17)score in the experi-mental group was also lower than that in the control group(P<0.05).The Olanzapine dosage,N-desmethyl Olan-zapine concentration,and serum prolactin levels in the experimental group were lower than those in the control group(P<0.05).The incidence of adverse reactions in both groups was similar(P>0.05).Conclusion Combined treat-ment with Olanzapine and Fluvoxamine can improve the negative symptoms and depressive symptoms in schizophrenia patients with depression.The combined treatment also reduced the dosage of Olanzapine and N-desmethyl Olanzapine,thus lowering serum prolactin levels.The treatment is safe and effective.
