摘要
为评价老年胆结石手术治疗的临床效果,选取2021年4月—2022年5月贵州省遵义市习水县中医医院收治的100例老年胆结石患者作为研究对象,随机分为两组,分别为参比组和应用组,每组各50例。其中,参比组患者实施传统开放手术治疗,应用组患者实施腹腔镜手术治疗,评价两种手术方法效果、手术指标、总胆红素(total bilirubin,TBIL)、丙氨酸氨基转移酶(alanine aminotransferase,ALT)水平、并发症发生率和炎性因子水平。结果显示,应用组治疗总有效率高于参比组(P<0.05)。应用组手术时间、切口长度、出血量、肛门排气时间、住院时间均优于参比组(P<0.05)。应用组TBIL、ALT水平低于参比组(P<0.05)。应用组并发症发生率低于参比组(P<0.05)。手术前,两组患者炎性因子水平差异无统计学意义(P>0.05);手术后7d,应用组血清肿瘤坏死因子-a(TNF-a)、白细胞介素-6(IL-6)水平低于参比组,应用组白细胞介素-10(IL-10)水平高于参比组(P<0.05)。研究发现,老年胆结石腹腔镜手术具有手术时间短、创口小、出血量少的特点,有利于减少炎症刺激,降低并发症发生风险,疗效确切,值得推广应用。
Evaluate the clinical efficacy of surgical treatment for elderly gallstones.100 elderly patients with gallstones admitted to Xishui Hospital of Traditional Chinese Medicine in Zunyi City,Guizhou Province from April 2021 to May 2022 were selected as the research subjects and randomly divided into two groups,with 50 cases in each group.Among them,the reference group patients received traditional treatment,while the application group patients received laparoscopic surgery.The effectiveness of the two surgical methods,surgical indicators,total bilirubin,alanine aminotransferase levels,and complication rate were evaluated.The results showed that the total effective rate of the application group was higher than that of the reference group(P<0.05).The application group had better surgical time,incision length,bleeding volume,anal exhaust time,and hospital stay than the control group(P<0.05).The levels of total bilirubin and alanine aminotransferase in the application group were lower than those in the control group(P<0.05).The incidence of complications in the application group was lower than that in the control group(P<0.05).Before surgery,there was no statistically significant difference in the levels of inflammatory factors between the two groups of patients(P>0.05);7 days after surgery,the serum levels of tumor necrosis factor-a(TNF-a)and interleukin-6(IL-6)in the application group were lower than those in the control group,while the levels of interleukin-10(IL-10)in the application group were higher than those in the control group(P<0.05).Research has found that laparoscopic surgery for elderly gallstones has the characteristics of short surgical time,small wound size,and low bleeding,which is beneficial for reducing inflammatory stimulation,reducing the risk of complications,and has a definite therapeutic effect.It is worth promoting and applying.
