摘要
目的 评价国产固定平台膝关节单髁置换术(UKA)治疗膝关节前内侧骨关节炎的近期临床疗效。方法 回顾性分析15例行国产固定平台UKA的膝关节前内侧骨关节炎患者的临床资料,记录患者的手术时间、术中出血量、术后并发症,并比较患者手术前后视觉模拟评分法(VAS)评分及美国特种外科医院膝关节(HSS)评分。结果 15例患者均获得随访,随访6~12个月。1例于术后6个月出现胫骨假体松动下沉,但膝关节疼痛仍较术前好转,继续观察;1例于术后1年内多次出现活动时膝关节交锁症状,每次均行局部按摩后好转,考虑膝关节不稳定(内侧副韧带松弛),继续观察。无切口感染、肺栓塞、深静脉血栓衬垫脱位等并发症。平均手术时间为(95±20)min,平均手术出血量为(80±15)ml。患者术前VAS评分为(6.2±1.5)分,术后为(1.8±0.6)分;患者术前膝关节HSS评分为(54.0±7.3)分,术后为(87.4±6.3)分。患者术后VAS评分明显低于术前,膝关节HSS评分明显高于术前,差异均有统计学意义(P<0.05)。结论 国产固定平台UKA治疗膝关节内侧骨关节炎的近期疗效满意。
Objective To evaluate the recent efficacy of domestic fixed platform unicompartmental knee arthroplasty(UKA)in the treatment of anteromedial osteoarthritis of the knee.Methods The clinical data of 15 patients with anteromedial osteoarthritis of the knee undergoing domestic fixed platform UKA were retrospectively analyzed,and the operative time,intraoperative blood loss,and postoperative complications of the patients were recorded,and the visual analogue scale(VAS)score and the Hospital for Special Surgery(HSS)score before and after surgery were compared.Results All 15 patients were followed up for 6-12 months.1 case showed loose and sinking of tibial prosthesis at 6 months after surgery,but the knee pain was still better than before surgery,and continued to be observed;1 case showed symptoms of locking of the knee during activities for many times in the first year after surgery,and each time it was improved after local massage,and the knee joint was considered to be unstable(laxity of medial collateral ligaments),and continued to be observed.There were no incisional infection,pulmonary embolism,deep vein thrombosis pad dislocation and other complications.The average operative time was(95±20)min,and the average intraoperative blood loss was(80±15)ml.VAS score was(6.2±1.5)points before surgery and(1.8±0.6)points after surgery.The knee HSS score was(54.0±7.3)points before surgery and(87.4±6.3)points after surgery.After surgery,VAS score was significantly lower than that before surgery,and the knee HSS score was significantly higher than that before surgery.The differences were statistically significant(P<0.05).Conclusion The recent efficacy of domestic fixed platform UKA is satisfactory in the treatment of anteromedial osteoarthritis of the knee.
