miR-146 a、sIL-2 R、hs-CRP在Hp相关性胃炎疗程中动态变化及意义

摘要 目的探讨微小RNA-146a(miR-146a)、可溶性白细胞介素-2受体(sIL-2R)、超敏C反应蛋白(hs-CRP)在幽门螺旋杆菌(Hp)相关性胃炎疗程中动态变化及意义。方法选取2021年2月至2022年2月开封市中心医院收治的166例Hp相关性胃炎患者为研究对象,根据治疗14 d后临床疗效将患者分为有效组(125例)和无效组(41例)。比较两组治疗前、治疗3 d、7 d、14 d后血清miR-146a、sIL-2R、hs-CRP水平,分析血清miR-146a、sIL-2R、hs-CRP水平间相关性和治疗3 d、7 d后血清各指标对治疗无效的危险度。结果治疗3 d、7 d、14 d后无效组血清miR-146a、sIL-2R、hs-CRP水平较有效组高(P<0.05);治疗3 d、7 d、14 d后血清miR-146a水平与sIL-2R、hs-CRP水平呈正相关,血清sIL-2R水平与hs-CRP水平呈正相关(P<0.05);Logistic回归分析显示,血清miR-146a、sIL-2R、hs-CRP水平是Hp相关性胃炎患者治疗无效的影响因素(P<0.05);治疗3 d、7 d后血清miR-146a、sIL-2R、hs-CRP高水平患者治疗无效的危险度高于低水平(P<0.05)。结论血清miR-146a、sIL-2R、hs-CRP水平可影响Hp相关性胃炎的疗效,动态监测其水平变化对临床治疗方案的制定起到指导作用。 【Objective】To investigate the dynamic changes and significance of microRNA-146a(miR-146a),soluble interleukin-2 receptor(sIL-2R),hypersensitive C-reactive protein(hs-CRP)in the course of Helicobacter pylori(Hp)-associated gastritis.【Methods】One hundred and sixty-six patients with Hp-associated gastritis admitted to our hospital from February 2021 to February 2022 were selected for the study,and the patients were divided into effective group(125 cases)and ineffective group(41 cases)according to the clinical efficacy after 14-day treatment.The serum miR-146a,sIL-2R、hs-CRP levels before treatment,after 3-day,7-day and 14-day treatment were compared between the two groups,and the correlation between serum miR-146a,sIL-2R,hs-CRP levels and the risk of ineffective treatment by each index of serum after 3-day and 7-day treatment were analyzed.【Results】Serum miR-146a,sIL-2R,hs-CRP levels were higher in the ineffective group than in the effective group after 3-day,7-day and 14-day treatment(P<0.05).Serum miR-146a levels were positively correlated with sIL-2R,hs-CRP levels after 3-day,7-day and 14-day treatment,and serum sIL-2R levels were positively correlated with hs-CRP levels(P<0.05).Logistic regression analysis showed that serum miR-146a,sIL-2R,hs-CRP levels were influential factors for treatment ineffectiveness in patients with Hp-associated gastritis(P<0.05).The risk of treatment ineffectiveness was higher in patients with high levels of serum miR-146a,sIL-2R,hs-CRP than low levels after 3-day and 7-day treatment(P<0.05).【Conclusion】Serum miR-146a,sIL-2R,hs-CRP levels can affect the efficacy of Hp-associated gastritis,and dynamic monitoring of their level changes can play a guiding role in the development of clinical treatment plans.
