摘要
目的观察金雀根汤联合醋酸泼尼松片治疗狼疮性肾炎(LN)的临床疗效。方法将92例LN患者按照随机数字表法分为2组,对照组46例予以标准疗程的激素治疗,治疗组46例在对照组治疗基础上加服金雀根汤,均以1个月为1疗程,共观察6个疗程。比较2组治疗前后中医证候评分、血清白细胞介素6(IL-6)、IL-18、IL-34、C反应蛋白(CRP)、免疫球蛋白G(IgG)、补体C3、C4、红细胞沉降率(ESR)、24 h尿蛋白定量(UPR)水平,并统计2组临床疗效及不良反应。结果治疗组总有效率91.30%(42/46),对照组总有效率76.09%(35/46),治疗组临床疗效优于对照组(P<0.05)。2组治疗后中医证候各项评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后血清IL-6、IL-18、IL-34、CRP水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后C3均较本组治疗前升高(P<0.05),24 hUPR均降低(P<0.05),且治疗组治疗后24 hUPR低于对照组(P<0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论LN患者在标准激素治疗中加用金雀根汤有利于提高临床疗效,可下调炎症因子水平,改善24 hUPR水平,且安全性较高,值得进一步研究探讨。
Objective To observe the therapeutic effect of Jinquegen Decoction combined with prednisone acetate therapy on lupus nephritis(LN).Methods Totally 92 LN patients were randomized 1∶1 to receive a standard course of hormone treatment(the control group)or Jinqiaogen Decoction+hormone therapy(the treatment group)one month was as a course of treatment,a total of 6 courses of observation.The key observation was TCM syndrome score,the levels of inflammatory factors including interleukin-6(IL-6),IL-18,and IL-34,and the levels of C-reactive protein(CRP),immunoglobulin G(IgG),complement C3,C4,erythrocyte sedimentation rate(ESR)and 24-hour urinary protein(UPR).Clinical efficacy was conducted.Results After treatment,the total effective rate in the treatment group was better than that in the control group(91.30%[42/46]vs 76.09%[35/46],P<0.05).The TCM syndromes scores in both groups were significantly decreased(P<0.05),the decrease in the treatment group was more significant(P<0.05).The levels of IL-6,IL-18,IL-34 and CRP in the both groups were significantly decreased(P<0.05),the decrease was more pronounced in the treatment group than in control group(P<0.05).The levels of complement C3 in both groups were significantly higher than those before treatment(P<0.05).The 24 hUPR was significantly lower after treatment(P<0.05),and the 24 hUPR in the treatment group was significantly lower than that in the control group(P<0.05).The incidences of adverse reactions in the two groups were similar(P>0.05).Conclusion For LN patients,Jinquegen Decoction combined with hormone therapy is beneficial for improving clinical efficacy,down-regulating inflammatory factor levels,improving 24 hUPR levels,and with has high safety.It is worth further research and exploration.
