摘要
目的比较双带襻钢板与锁骨远端解剖锁定钢板(LAP)在NeerⅡ型锁骨远端骨折(DCF)患者临床治疗中的效果。方法选取2018年12月—2021年12月该院收治的92例NeerⅡ型DCF患者,根据术式的不同进行分组,42例采用双带襻钢板治疗的患者为双带襻钢板组,50例采用LAP治疗的患者为LAP组。记录二组围术期相关指标,并与于术后6个月,比较二组临床疗效、患肩肩关节功能情况,记录术后并发症的发生情况。结果术后6个月时,双带襻钢板组的整体临床疗效明显优于LAP组(P<0.05);双带襻钢板组患者的手术时间、术中出血量均高于LAP组(P<0.05),住院时间与LAP组比较差异无统计学意义(P>0.05);双带襻钢板组在疼痛、ADL、ROM及肌力各方面的评分均高于LAP组(P<0.05);二组患者中均无切口感染发生,部分患者出现肩部剧烈疼痛、肩峰撞击及内固定物断裂或松动情况,双带襻钢板组的术后并发症总发生率为4.76%,低于LAP组的20.00%(P<0.05)。结论尽管双带襻钢板治疗NeerⅡ型DCF的手术时间较长,但其临床疗效较优,且患者术后患肩肩关节功能恢复情况更佳,值得临床进一步研究。
Objective To compare the clinical curative effect of double Endobutton and locking anatomical plate(LAP)on patients with Neer II type distal clavicle fractures(DCF).Methods A total of ninety-two patients with Neer II type DCF admitted to the hospital were enrolled between December 2019 and December 2021.According to different surgical methods,they were divided into double Endobutton group(42 cases,double Endobutton)and LAP group(50 cases,LAP).The perioperative indexes in the two groups were recorded.At 6 months after surgery,clinical curative effect and function of effected shoulders in the two groups were compared.The occurrence of postoperative complications was recorded.Results At 6 months after surgery,overall clinical curative effect in double Endobutton group was significantly better than that in LAP group(P<0.05),operation time and intraoperative blood loss were higher than those in LAP group(P<0.05).However,there was no significant difference in hospitalization time between the two groups(P>0.05).The scores of pain,ADL,ROM and muscle strength in double Endobutton group were higher than those in LAP group(P<0.05).There was no incision infection in either group.There were some patients with severe shoulder pain,acromion impact and internal fixation fracture or loosening.The total incidence of postoperative complications in double Endobutton group was lower than that in LAP group(4.76%vs.20.00%,P<0.05).Conclusion Although the operation time of double Endobutton is longer in the treatment of Neer II type DCF,its clinical curative effect and postoperative function recovery of effected shoulders are better.
