摘要
目的探讨四逆汤对脓毒症休克患者胃肠道功能影响的研究。方法选取杭州市第三人民医院急诊监护室及重症监护室2020年1月—2022年1月所收治的共85例脓毒症休克患者作为研究对象,按随机数字表法将其分为研究组(43例)与参照组(42例),研究组在常规西医治疗基础上给予四逆汤穴位贴敷联合双歧杆菌三联活菌散口服或鼻饲,参照组在常规西医治疗基础上给予滑石粉穴位贴敷联合双歧杆菌三联活菌散口服或鼻饲。研究比较两组患者的胃残余量、急性胃肠损伤超声检查评分(acute gastrointestinal injury ultrasonography score,AGIUS评分)、肠内营养(enteral nutrition,EN)达标时间。结果治疗后,两组患者胃残余量、AGIUS评分均低于治疗前,差异具有统计学意义(P<0.05)。治疗后,研究组AGIUS评分、EN达标时间低于参照组,差异具有统计学意义(P<0.05),但两组胃残余量相仿,差异不具有统计学意义(P>0.05)。结论四逆汤可改善脓毒症休克患者的胃肠道功能,起到较好的保护作用。
Objective To explore the effect of Sini Decoction(四逆汤)on gastrointestinal function of patients with septic shock.Methods A total of 85 patients with septic shock admitted to the emergency and intensive care units of the hospital from January 2020 to January 2022 were selected as the research objects.They were divided into research group and reference group according to random number table method.A total of 43 patients in the research group were treated with acupoint application with Sini Decoction combined with Bifidobacterium triple viable powder oral administration or nasal feeding on the basis of conventional Western medicine treatment and 42 patients in the reference group were treated with talcum powder acupoint application combined with Bifidobacterium triple viable powder oral administration or nasal feeding on the basis of conventional Western medicine treatment.Gastric residual volume,acute gastrointestinal injury ultrasound score(AGIUS score)and enteral nutrition(EN)standard time were compared between the two groups.Results After treatment,the gastric residual volume and AGIUS score of the two groups were lower than those before treatment,and the differences were statistically significant(P<0.05).After treatment,the AGIUS score and EN standard time of the research group were lower than those of the reference group and the differences were statistically significant(P<0.05),but the gastric residual volume of the two groups was similar,and the differences were not statistically significant(P>0.05).Conclusion Sini Decoction can improve the gastrointestinal function of patients with septic shock and has a good protective effect.
