摘要
目的探究益气活血泻浊方加减对尿毒症患者血液透析(HD)治疗疗效及炎症反应的影响。方法选取2020年1月—2021年6月浙江中医药大学附属湖州中医院收治的80例行HD治疗的尿毒症患者为研究对象,依据随机数字表法将患者分为对照组(40例)和观察组(40例),对照组给予常规治疗,观察组在上述基础实施益气活血泻浊方加减治疗,连续治疗4周,比较两组患者临床疗效、中医证候积分、炎症因子水平、残余肾功能(RRF)、24 h尿量。结果观察组临床有效率(92.50%,37/40)较对照组(72.50%,29/40)显著高(P<0.05);治疗前,两组患者中医证候积分(面色白光白、腹胀纳呆、倦怠乏力、小便不利)、炎症因子水平[白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)]比较差异无统计学意义(P>0.05),治疗后,两组患者中医证候积分、炎症因子水平均下降,且观察组较对照组更低(P<0.05);治疗前,两组患者RRF、24 h尿量比较差异无统计学意义(P>0.05),治疗后,两组患者上述指标均下降,但观察组高于对照组(P<0.05)。结论尿毒症HD患者应用益气活血泻浊方加减治疗,疗效显著,可有效改善临床症状,降低炎症因子水平,保护残余肾功能。
Objective To investigate the influence of Yiqi Huoxue Xiezhuo Decoction(益气活血泻浊方)on the curative effect of hemodialysis(HD)and inflammatory response in patients with uremia.Methods A total of 80 patients with uremia who underwent HD in the hospital from January 2020 to June 2021 were selected as the research subjects.The patients were divided into control group(40 cases)and observation group(40 cases)according to the random number table method.The patients in the control group were given conventional treatment and those in the observation group were treated with Yiqi Huoxue Xiezhuo Decoction on this basis.Both groups underwent 4 consecutive weeks of treatment.The clinical effects,TCM symptom scores,inflammatory factor levels,residual renal function(RRF),and 24 h urine output were compared between the two groups.Results The clinical response rate in the observation group(92.50%,37/40)was significantly higher than that in the control group(72.50%,29/40)(P<0.05).The TCM symptom scores and the levels of inflammatory factors[interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α)]were similar in the two groups before treatment(P>0.05).After treatment,TCM symptom scores and the levels of inflammatory factors decreased,which were lower in the observation group than those in the control group(P<0.05).The values of RRF and 24 h urine output showed no significant difference between the two groups before treatment(P>0.05).After treatment,the values of RRF and 24 h urine output in the two groups decreased and the values in the observation group were higher than those in the control group(P<0.05).Conclusion Yiqi Huoxue Xiezhuo Decoction is effective in the treatment of patients undergoing HD for uremia,and can effectively improve the clinical symptoms,lower the levels of inflammatory factors and protect the residual renal function.
