摘要
目的探讨D-二聚体(DD)、N端脑钠肽前体(NT-proBNP)水平与经PCI治疗的急性心肌梗死(AMI)患者近期临床预后的相关性分析。方法选取2019年1月—2020年1月肇庆市第一人民医院收治的经PCI治疗的AMI患者300例,随访1年,记录术后主要不良心血管事件(MACE),检测血清DD及NTproBNP水平,并分析其与MACE的相关性。结果与预后良好组比较,预后不良组随访期间血清NTproBNP及DD水平较高(P<0.05);NT-proBNP联合DD诊断AMI患者发生MACE的曲线下面积为0.859(95%CI:0.778~0.906,P<0.05),约登指数为0.489,敏感度为93.58%,特异度为83.71%;NT-proBNP联合DD诊断AMI患者发生MACE的敏感度和特异度均高于NT-proBNP及DD单独诊断(P<0.001);NT-proBNP、DD与AMI术后发生MACE相关(P<0.05)。结论NT-proBNP及DD水平升高与经PCI治疗的AMI患者不良预后密切相关,监测AMI患者PCI术后NT-proBNP及DD水平对评估预后有重要作用。
Objective To investigate the correlation between D-dimer and B-type natriuretic peptide(NT-proBNP)levels and the recent clinical prognosis of patients with acute myocardial infarction(AMI)treated with PCI.Methods 300 AMI patients treated by PCI in our hospital from January 2019 to January 2020 were selected,followed up for one year,recorded major adverse cardiovascular events(MACE)after surgery,detected the levels of serum DD and NT-proBNP,and analyzed the correlation between them and MACE.Results Compared with the good prognosis group,the serum NT-proBNP and DD levels in the poor prognosis group were higher during the follow-up period(P<0.05).The area under the curve for the diagnosis of MACE in AMI patients by NT-proBNP combined with DD was 0.859(95%CI:0.778-0.906,P<0.05).the Yoden index was 0.489,the sensitivity was 93.58%,and the specificity was 83.71%;The sensitivity and specificity of NT-proBNP combined with DD in diagnosing MACE in AMI patients were higher than those of NT-proBNP and DD alone(P<0.001);NT-proBNP and DD were correlated with MACE after AMI(P<0.05).Conclusion The increase of NT-proBNP and DD levels is closely related to the poor prognosis of AMI patients treated by PCI.Monitoring NT-proBNP and DD levels after PCI is important for evaluating the prognosis of AMI patients.
