摘要
目的探讨不同头低足高卧位对下肢深静脉血流动力学的影响。方法96例健康志愿者采用自身对照,依次测量平卧位、头低足高截石位及头低足高小腿下垂式反弓分腿卧位时左侧股静脉内径、平均血流速度,并计算平均血流量;采用自行设计的舒适度量表调查志愿者完成测试后的舒适度。结果头低足高截石位时志愿者的股静脉内径增粗,平均血流速度及血流量均减少(P<0.001);头低足高小腿下垂式反弓分腿卧位舒适度高于头低足高截石位,差异有统计学意义(P<0.05)。结论头低足高小腿下垂式反弓分腿卧位有利于血液回流,可能降低术后下肢深静脉血栓发生的风险,增加患者舒适度。
Objective To compare the effects of different operative positions on deep venous blood flow mechanics of lower limbs during robot-assisted laparoscopic urinary tract surgery.Methods 96 self-controlled healthy volunteers were included.The internal diameter and average blood flow velocity of the left femoral vein were measured in the supine position,lithotomy position as well as head low and foot high leg drooping reverse bow leg splitting position,and the average blood flow was calculated.Self-designed comfort scale was used to investigate the comfort level of the subjects after completing the test.Results Compared with lithotomy position,the recumbent position presents larger femoral vein diameter.While the average blood flow velocity and blood flow were decreased(P<0.001).Meanwhile,the comfort level of recumbent position was higher than lithotomy position(P<0.05).Conclusion The recumbent position is beneficial to lower limbs venous blood reflux,which may reduce the risk of postoperative DVT and increase the comfort level of patients.
