福建省2018—2021年副溶血性弧菌的病原学特征 被引量:5

Etiological characteristics of Vibrio parahaemolyticus in Fujian Province,2018-2021

摘要 目的了解福建省副溶血性弧菌(VP)病原学特征及耐药情况,为食源性疾病暴发溯源提供参考。方法采用抗血清凝集、实时荧光PCR和微量肉汤稀释法,对2018—2021年福建省分离的96株VP进行血清型、毒力基因(tlh、tdh、trh)和耐药性检测。结果96株中的病例株90株分为13种血清型,O10∶K4(37.8%)和O3∶K6(34.4%)是优势血清型;新冠疫情暴发前后VP血清型分布差异有统计学意义(Fisher确切χ2=49.57,P=0.000);暴发前O3∶K6血清型占比最高,达53.1%(17/32),暴发后O10∶K4血清型占比最高,达58.6%(34/58);90株中93.3%(84株)携带tdh基因;5.6%(5株)携带trh基因,其血清型是O1∶KUT、O4∶K68和O11∶K5;1.1%(1株)O1∶K56均不携带tdh和trh基因;6株暴发事件食品和环境分离株的血清型与病例株均不同,1株O10∶KUT携带tdh;头孢唑林、氨苄西林和多粘菌素E的耐药率分别为97.9%、66.7%和45.8%;多重耐药株占9.4%(9/96)。结论福建省2018—2021年分离的VP共96株,O10∶K4(37.8%)和O3∶K6(34.4%)是优势血清型;2020年首次出现O10∶K4血清型,并与O3∶K6成为腹泻病例的优势血清型;同一起暴发事件中出现多种血清型且致病基因不同的菌株,提示暴发事件中食品污染源头溯源较为复杂,需要采取多种分型方法加以鉴别。 Objective To investigate the pathogenic characteristics and drug resistance of Vibrio parahaemolyticus(VP)in Fujian Province,and to provide references for tracing outbreaks of foodborne disease.Methods Antiserum agglutination,Realtime PCR,and microbroth dilution were used to detect serotype,virulence gene(tlh,tdh,trh),and drug resistance of 96 VPs isolated from Fujian Province from 2018 to 2021.Results Ninety out of the 96 case strains were divided into 13 serotypes,with O10∶K4(37.8%)and O3∶K6(34.4%)being the dominant serotypes.There were statistically significant differences in the distribution of VP serotypes before and after the outbreak of COVID19(Fisher exactχ^(2)=49.57,P=0.000).O3∶K6 serotype accounted for the highest proportion before the outbreak of COVID19,reaching 53.1%(17/32),while O10∶K4 serotype accounted for the highest proportion after the outbreak,reaching 58.6%(34/58).Among the total 90 strains,93.3%(84 strains)carried the tdh gene and 5.6%(5 strains)carried the trh gene,and their serotypes were O1∶KUT,O4∶K68,and O11∶K5.Meanwhile,1.1%(1 strain)was O1∶K56 which did not carry tdh and trh genes.The serotypes of isolates from six food and environment outbreak were different from those case strains,among which one strain of O10∶KUT carried tdh gene.The resistance rates of cefazolin,ampicillin,and polymyxin E were 97.9%,66.7%,and 45.8%,respectively.Multidrugresistant strains accounted for 9.4%(9/96).Conclusion Among the total of 96 VP strains isolated from 2018 to 2021 in Fujian Province,O10∶K4(37.8%)and O3∶K6(34.4%)were the dominant serotypes.Serotype O10∶K4,which first emerged in 2020,was the dominant serotype in diarrhea cases together with O3∶K6.The presence of strains with multiple serotypes and different pathogenic genes in the same outbreak often suggests that the source of food contamination in an outbreak is complex to trace and requires multiple typing methods to identify.
