摘要
目的分析住院药房临床不合理医嘱用药情况及干预效果。方法选取广东省人民医院住院药房2019年5月—2020年4月以及2020年5月—2021年4月共2年住院药房不合理医嘱进行统计分析,对比医嘱差错情况。结果2年不合理医嘱类型中用法用量不适宜为首要类型,其次为重复医嘱、配伍禁忌。2020年5月—2021年4月的差错干预率为0.046%,2019年5月—2020年4月为0.068%,两时段对比总差错率下降了32.35%。结论通过一定的干预措施,药师对住院医嘱进行长期监测和干预,发现不合理医嘱及时与临床医生沟通修改,能够有效降低不合理用药情况发生率,提高临床合理用药水平,保障患者用药安全。
Objective To analyze the clinical unreasonable medication advice and intervention effect in the inpatient pharmacy.Methods The unreasonable medication advices of inpatient pharmacy in our hospital from May 2019 to April 2020 and From May 2020 to April 2021 were selected for statistical analysis.Results From 2019-2021,inappropriate use and dosage was the first type of unreasonable medication advices followed by repeated medical orders,and the third was incompatibility.The error intervention rate in the second year was 0.046%,while the first year was 0.068%.the total error rate in the two periods decreased by 32.35%.Conclusion Through certain intervention measures,pharmacists carry out long-term monitoring and intervention on inpatient medical orders,and timely communicate with clinicians to modify unreasonable medication advice,which can effectively reduce the incidence of irrational drug use,better improve clinical rational drug use,so as to ensure the safety of patients'drug use.
