摘要
目的分析脑卒中后肩手综合征患者康复治疗效果的影响因素。方法本研究纳入2020年3月至2022年2月在南阳南石医院接受为期3个月康复治疗的脑卒中后肩手综合征患者共78例,根据康复治疗效果将患者分为有效组(n=66)及无效组(n=12),收集患者病例资料,包括患者年龄、性别、身体质量指数(body mass index,BMI)、文化程度、病人与看护者关系、肩痛程度、卒中部位、焦虑、抑郁、认知功能障碍、康复介入时机及康复治疗配合度,多因素logistic回归分析康复治疗效果的影响因素。结果康复治疗的有效率为84.62%(66/78)。康复治疗有效组与无效组之间,年龄、文化程度、肩痛程度、焦虑、抑郁、认知功能障碍、康复介入时机及康复治疗配合度存在显著性差异(P<0.05);而性别、BMI、病人与看护者的关系及卒中部位无显著性差异(P>0.05)。多元logistic回归分析发现,患者年龄、文化程度、肩痛程度、焦虑、抑郁、认知功能障碍、康复介入时机及康复治疗配合度为康复治疗效果的独立影响因素(P<0.05)。结论脑卒中后肩手综合征患者康复治疗效果受多种主客观因素影响,临床上加强对康复治疗的宣传、及时疏导患者不良情绪及对患者进行早期康复治疗等干预措施有望提高康复治疗效果。
Objective To analyze the influencing factors of rehabilitation therapy in patients with post-stroke shoulder-hand syndrome.Methods A total of 78 patients with post-stroke shoulder-hand syndrome who received 3-month rehabilitation therapy in our hospital from March 2020 to February 2022 were included in this study.After 3-month rehabilitation therapy,the patients were divided into effective group(n=66)and ineffective group(n=12).The data of all patients were collected,including age,gender,BMI,education level,relationships between patients and caregivers,degree of shoulder pain,stroke site,anxiety,depression,cognitive dysfunction,rehabilitation intervention time,and rehabilitation treatment cooperation degree,and multiple logistic regression analysis was conducted to analyze the influencing factors of rehabilitation treatment.Results The effective rate of rehabilitation treatment was 84.62%(66/78).There were significant differences in age,education level,degree of shoulder pain,anxiety,depression,cognitive dysfunction,rehabilitation intervention time,and rehabilitation treatment cooperation degree between effective and ineffective groups(P<0.05).There were no significant differences in gender,BMI,relationships between patients and caregivers,and stroke site(P>0.05).Multiple logistic regression analysis showed that age,education level,degree of shoulder pain,anxiety,depression,cognitive dysfunction,rehabilitation intervention time,and rehabilitation treatment cooperation degree were independent factors influencing the effect of rehabilitation treatment(P<0.05).Conclusion The effects of rehabilitation therapy in patients with post-stroke shoulder-hand syndrome were affected by multiple subjective and objective factors.The effect of rehabilitation therapy for patients with post-stroke shoulder-hand syndrome is affected by a variety of subjective and objective factors.Intervention measures such as strengthening the publicity of rehabilitation therapy,timely channeling of patients'bad mood,and early rehabilitation thera
