摘要
目的 评价玻璃体切除术联合后Tenon囊下注射曲安奈德治疗增生性糖尿病视网膜病变(proliferative diabetic retinopathy,PDR)的临床疗效。方法 回顾性分析2018年4月—2021年3月在福建医科大学附属南平第一医院治疗的PDR患者80例(80眼)的临床资料。以入院顺序为依据分为对照组(40例)与观察组(40例)。对照组给予玻璃体切除术,观察组在对照组基础上联合后Tenon囊下注射曲安奈德。随访6个月,对比分析术后视力、黄斑中心凹厚度、眼压,记录两组并发症发生率。结果 两组术前及术后6个月视力比较,差异无统计学意义(P>0.05)。两组术后1周,1个月、3个月、6个月视力较术前均有不同程度的提高,且术后1周,1个月、3个月观察组视力改善优于对照组,差异有统计学意义(P<0.05)。术后1周,1个月、3个月观察组黄斑中心凹厚度均优于对照组,差异有统计学意义(P<0.05),术后6个月,两组黄斑中心凹厚度比较,差异无统计学意义(P>0.05)。两组患者术前、术后1周,1个月、3个月、6个月眼压比较,差异无统计学意义(P>0.05)。观察组术后瞳孔区膜样渗出发生率明显低于对照组,差异有统计学意义(P<0.05),而两组玻璃体积血发生率比较,差异无统计学意义(P>0.05)。结论 玻璃体切除术后联合后Tenon囊下注射曲安奈德治疗PDR,能够改善视力,减少黄斑水肿,抑制眼内炎症,对眼压影响小,安全性高。
Objective To evaluate the clinical effect of vitrectomy combined with posterior Tenon subcapsular injec⁃tion of triamcinolone in the treatment of proliferative diabetic retinopathy(PDR).Methods Retrospective analysis of clinical data of 80 PDR patients(80 eyes)treated at Nanping First Hospital Affiliated to Fujian Medical University from April 2018 to March 2021.Divide into a control group(40 cases)and an observation group(40 cases)based on the order of admission.The control group received vitrectomy,while the observation group was combined with poste⁃rior Tenon subcapsular injection of triamcinolone acetonide on the basis of control group.Follow up for 6 months,com⁃pare and analyze postoperative visual acuity,macular fovea thickness,and intraocular pressure,and record the inci⁃dence of complications between the two groups.Results There was no statistically significant difference in visual acu⁃ity between the two groups before and 6 months after surgery(P>0.05).Both groups showed varying degrees of im⁃provement in visual acuity at 1 week,1 months,3 months,and 6 months after surgery,and the observation group had better visual acuity improvement than the control group at 1 week,1 months,and 3 months after surgery,the differ⁃ence was statistically significant(P<0.05).At 1 week and 1 months and 3 months after surgery,the thickness of the macular fovea in the observation group was better than that in the control group,the difference was statistically signifi⁃cant(P<0.05).At 6 months after surgery,there was no statistically significant difference in the thickness of the macu⁃lar fovea between the two groups(P>0.05).There was no statistically significant difference in intraocular pressure be⁃tween the two groups of patients before and 1 week,1 months,3 months,and 6 months after surgery(P>0.05).The in⁃cidence of postoperative membranous exudation in the pupil area of the observation group was significantly lower than that of the control group,the difference was statistically significant(P<0.05).Howe
