摘要
目的 探讨髓过氧化物酶(myeloperoxidase, MPO)和超氧化物歧化酶(superoxide dismutase, SOD)在脑卒中患者病情发展过程中的水平变化及预后评估作用。方法 选取湖南省脑科医院2021年9月-2022年4月,收治的55例脑卒中患者(其中42例缺血性卒中患者、13例出血性卒中患者)为研究对象,收集并追踪患者入院后5 d的血清样本,检测其MPO、 SOD血清表达水平,统计分析每日MPO、 SOD水平对病情严重程度及预后评估;采用ROC曲线分析脑卒中患者入院当日MPO、SOD水平对脑卒中早期诊断效能。结果 与健康对照组比较,入院当日脑卒中患者血清MPO、SOD水平明显降低(P<0.01);利用ROC曲线分析结果显示,MPO和SOD预测脑卒中的曲线下面积(area under the curve, AUC)分别为0.9921和0.8408;监测血清MPO水平变化得知,脑卒中患者病情严重程度与入院后次日MPO水平增高值相关(P<0.01),且MPO增高程度愈高,患者预后愈差(P=0.006);监测血清SOD水平变化得知,脑卒中患者预后状况与入院当日血清SOD值相关(P=0.017)。结论 血清MPO、SOD对脑卒中的早期诊断和病情评估可提供参考价值。
Objective To explore the changes of myeloperoxidase(MPO)and superoxide dismutase(SOD)levels during the progression of stroke patients and the roles of them in prognosis evaluation.Methods We selected 55 stroke patients(including 42 patients with ischemic stroke and 13 patients with hemorrhagic stroke)hospitalized in Brain Hospital of Hunan Province from September 2021 to April 2022 to serve as the research subjects.Serum samples on 5 days after admission were collected and tracked,and then detected the levels of MPO and SOD.We statistically analyzed the roles of daily levels of MPO and SOD on evaluating the severity and prognosis of stroke.The ROC curve was used to analyze the efficacy of the stroke patients’MPO and SOD levels on the day of admission for early diagnosis of stroke.Results Compared with the healthy control group,the stroke patients’serum levels of MPO and SOD on the day of admission significantly decreased(P<0.01).The analysis results based on the ROC curve displayed that the area under the curves(AUCs)of MPO and SOD for predicting stroke were 0.9921 and 0.8408 respectively.Surveillance on the changes in the serum level of MPO revealed that the severity of stroke in the patients was correlated with the increase in the serum level of MPO on the second day after admission(P<0.01);moreover,the higher the increase in MPO,the poorer the prognosis of the patients(P=0.006).Surveillance on the changes in the serum level of SOD exhibited that the prognoses of the stroke patients were related to the serum level of SOD on the day of admission(P=0.017).Conclusion Serum MPO and SOD can provide reference values for early diagnosis and severity assessment of stroke.
