摘要
目的分析糖尿病足感染患者免疫功能和创面分泌物中血小板衍生生长因子BB(PDGF-BB)、碱性成纤维细胞生长因子(bFGF)水平与感染程度的关系。方法选取2020年6月-2022年6月武汉理工大学医院收治的82例糖尿病足感染患者为感染组,80例糖尿病足未感染患者为非感染组,另外选取50名健康体检者作为对照组;检测各组免疫功能、PDGF-BB、bFGF水平,分析免疫功能和创面分泌物中PDGF-BB、bFGF与感染程度的关系。结果感染组CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)、PDGF-BB、bFGF水平低于非感染组(P<0.05),非感染组以上指标低于对照组(P<0.05);Wagner分级3~4级患者CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)、PDGF-BB、bFGF低于Wagner分级1~2级患者(P<0.05);CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)、PDGF-BB、bFGF与Wagner分级呈负相关(P<0.05);趾肱指数(TBI)、糖化血红蛋白(HbALC)、CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)、PDGF-BB、bFGF、Wagner分级是糖尿病足患者感染的危险因素(P<0.05)。结论糖尿病足感染患者中CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)、PDGF-BB、bFGF水平较低,且CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)、PDGF-BB、bFGF表达情况与患者感染程度相关。
OBJECTIVE To analyze the relationship between immune function and the levels of platelet-derived growth factor BB(PDGF-BB)and basic fibroblast growth factor(bFGF)in trauma secretions and the degree of infection in patients with diabetic foot infection.METHODS Totally 82 patients with diabetic foot infection admitted to the Hospital of Wuhan University of Technology from Jun.2020 to Jun.2022 were selected as the infected group,80 diabetic foot patients without infection were selected as the non-infected group,and 50 healthy physical examiners were selected as the control group;immune function,PDGF-BB and bF GF levels of each group were tested,and the relationship between immune function and PDGF-BB,bF GF and infection level in trauma secretions and degree of infection were analyzed.RESULTS The levels of CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+),PDGF-BB,and bF GF in the infected group were lower than those in the non-infected group(P<0.05),while the above indicators in the non-infected group were lower than those in the control group(P<0.05);CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+),PDGF-BB,and bF GF were lower in patients with Wagner Grade 3 to 4 than in patients with Wagner grade 1 to 2(P<0.05);CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+),PDGF-BB,and bF GF were negatively correlated with Wagner grades(P<0.05);Toe brachial index(TBI),glycosylated hemoglobin(HbALC),CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+),PDGF-BB,bF GF,and Wagner grades were risk factors for infection in diabetic foot patients(P<0.05).CONCLUSION The levels of CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+),PDGF-BB and bFGF were low in patients with diabetic foot infection,and the expression levels of CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+),PDGF-BB and bFGF were correlated with the degree of infection.
