摘要
目的比较结核分枝杆菌/利福平耐药实时荧光定量核酸扩增检测技术(XpertMTB/RIF)与抗酸染色法对结核性腹膜炎的诊断价值。方法回顾性分析2019年2月至2022年2月河南省胸科医院检查的52例疑似结核性腹膜炎患者的临床资料,全部患者术前进行Xpert MTB/RIF试验以及抗酸染色法检测,以影像学改变特征及或腹膜病理学检查结果为标准评价两种影像学方法的诊断结果。结果52例疑似结核性腹膜炎患者中确诊43例。Xpert MTB/RIF检测灵敏度、准确度及特异度(97.67%,42/43;100.00%,9/9;98.08%,51/52)高于抗酸染色(69.77%,30/43;55.56%,5/9;67.31%,35/52),差异有统计学意义(P<0.05)。结论Xpert MTB/RIF试验灵敏度、准确度及特异度较高,可为结核性腹膜炎提供可靠的诊断依据。
Objective To compare the diagnostic value of Mycobacterium tuberculosis/rifampicin resistance real-time fluorescent quantitative nucleic acid amplification detection(Xpert MTB/RIF)and acid-fast staining method for tuberculous peritonitis.Methods Retrospective analysis was made on 52 patients with suspected tuberculous peritonitis who were examined in Henan Provincial Chest Hospital from February 2019 to February 2022.All patients underwent Xpert MTB/RIF test and acid-fast staining test before operation.The diagnostic results of the two imaging Methods were evaluated,with the characteristics of imaging changes and/or the results of peritoneal pathology as standard.Results Forty-three of 52 patients with suspected tuberculous peritonitis were definitely diagnosed.The sensitivity,accuracy and specificity of Xpert MTB/RIF test(97.67%,42/43;100.00%,9/9;98.08%,51/52)were significantly higher than those of acid-fast staining method(69.77%,30/43;55.56%,5/9;67.31%,35/52),and the difference was statistically significant(P<0.05).Conclusions The sensitivity,accuracy and specificity of Xpert MTB/RIF test are high,which provides a reliable basis for the diagnosis of tuberculous peritonitis.
