摘要
目的探讨导航护士在加速康复外科(enhanced recovery after surgery,ERAS)心脏病患者围手术期管理中的应用价值。方法选取2019年1月至2021年9月入住广元市第一人民医院的150例心脏手术患者作为研究对象,按入院顺序分为对照组和研究组各75例。对照组由责任护士常规落实ERAS护理方案,研究组由导航护士负责协调、督促和监控相关科室正确实施ERAS方案。比较两组患者ICU住院时间、关键措施执行率、术后疼痛、肺部感染发生率、排便情况和健康教育知识知晓率、护理满意度。结果两组患者均无肺部感染发生,ICU住院时间差异无统计学意义(P>0.05);与对照组相比,研究组便秘发生率和术后疼痛程度更低,关键措施执行率、健康教育知识知晓率、护理满意度更高,比较差异有统计学意义(P<0.05或0.001)。结论导航护士有更多的时间和精力专注于心脏围手术期患者的诊疗护理过程,保证ERAS措施执行的准确性、连续性,及时反馈和进行效果评价,使患者、家属更好地配合治疗护理,有利于防止护理并发症发生,促进患者康复。
Objective To explore the value of navigation nurse in perioperative management of patients with heart disease in enhanced recovery after surgery(ERAS).Methods 150 patients with cardiac surgery admitted to the First People's Hospital of Guangyuan from January 2019 to September 2021 were selected and divided into control group and experimental group according to admission order,with 75 cases in each group.In the control group,responsible nurses were responsible for the routine implementation of ERAS nursing program,and in the experimental group,navigation nurses were responsible for coordinating,supervising,and monitoring relevant departments to implement ERAS plan accurately.And then,the length of ICU hospitalization,implementation rate of key measures,postoperative pain,incidence of pulmonary infection,defecation,awareness rate of health education knowledge,and nursing satisfaction were compared between the two groups.Results There was no pulmonary infection in both groups,and there was no statistically significant difference in the length of ICU hospitalization(P>0.05).Compared with the control group,the incidence of constipation and postoperative pain in the experimental group were lower,and implementation rate of key measures,awareness rate of health education knowledge,and nursing satisfaction were higher,and differences were statistically significant(P<0.05 or 0.001).Conclusion Navigation nurses have more time and energy to focus on the diagnosis,treatment and nursing process of patients during the perioperative period of heart surgery,ensuring the accuracy and continuity of the implementation of ERAS measures,timely feedback and effect evaluation,so that patients and their families can better cooperate with treatment and nursing,which is conducive to preventing nursing complications and promoting patient recovery.
