摘要
目的:探讨围手术期外周静脉血中长链非编码RNA(long non-coding RNA,lncRNA)PAX8-AS1及微RNA(microRNA,miRNA)-1252-5p表达水平对胫骨骨折内固定术后骨折愈合的影响。方法:以2018年1月至2020年12月在宜兴市人民医院接受同一组医生完成的钢板内固定或髓内钉内固定手术治疗的胫骨骨折患者为研究对象。收集患者的性别、年龄、骨折类型、损伤类型、手术方式、吸烟史、饮酒史、高血压史、糖尿病史等一般资料,随访观察骨折愈合情况。采用荧光定量PCR检测术前及术后1周、2周、4周、8周时患者外周静脉血中lncRNA PAX8-AS1、miRNA-1252-5p的表达水平。根据术后4个月时的骨折愈合情况,将纳入的病例分为正常愈合组及延迟愈合组。比较2组患者的一般资料及手术前后外周静脉血中lncRNA PAX8-AS1、miRNA-1252-5p表达水平,分析影响骨折愈合的因素,评价外周静脉血中lncRNA PAX8-AS1、miRNA-1252-5p表达水平对骨折延迟愈合的预测价值。结果:(1)一般结果。共纳入250例患者,正常愈合组210例、延迟愈合组40例。延迟愈合组合并糖尿病比例、开放性骨折比例、高能量损伤比例均高于正常愈合组(χ^(2)=28.245,P=0.000;χ^(2)=24.881,P=0.000;χ^(2)=34.469,P=0.000)。(2)外周静脉血中lncRNA PAX8-AS1、miRNA-1252-5p表达水平检测结果。术后2周和术后4周时,延迟愈合组外周静脉血中lncRNA PAX8-AS1表达水平均高于正常愈合组(t=4.742,P=0.000;t=2.753,P=0.006),miRNA-1252-5p表达水平均低于正常愈合组(t=4.079,P=0.000;t=2.538,P=0.012)。(3)胫骨骨折内固定术后骨折愈合影响因素分析结果。广义估计方程分析结果显示,开放性骨折、高能量损伤及术后2周时外周静脉血中lncRNA PAX8-AS1表达水平升高和miRNA-1252-5p表达水平降低是胫骨骨折内固定术后骨折愈合的影响因素(β=29.382,Waldχ^(2)=8.589,P=0.000;β=22.771,Waldχ^(2)=9.766,P=0.000;β=57.418,Waldχ^(2)=13.735,P=0.000;β=-81.524,Waldχ
Objective:To explore the flects of expression levels of long non-coding RNA(IncRNA)PAX8-ASI and microRNA(miRNA)-1252-5p in perioperalive peripheral venous blood(PVB)on fracture healing afer intemal fixation for treatment of tibial fractures.Methods:'The patients who underwent intemal fixation with steel plate or intramedullary nail for treatment of tibial fractures by the same surgeons in Yixing People's Hospital from January 2018 to Deember 2020 were seleeted as the subjects,and their information including gender,age,fracture type,injuny type,surgery method,smoking history,drinking history,hypertension history and diabetes mellitus history was collected.The patients were fllowed up and the fracture healing was observed.The expression levels of IncRNA PAX8-AS1 and miRNA-1252-5p in PVB were detected by using fluorescent quantitative PCR method before the surgery and at postsurgical week 1,2,4 and 8.According to the fracure healing al posurgical month 4,the included patients were divided into normal healing group and delayed heal-ing group.The general data of patients and the presurgical and postsurgical expression levels of IncRNA PAX8-AS1 and miRNA-1252-5p in PVB were compared between the 2 groups.Moreover,the factors inluencing fracture healing were analyzed,and the value of the expression levels of IncRNA PAX8-ASI and miRNA-1252-5p in PVB in predicting delayed frnacture healing was evaluated.Results:①Two hundred and ffty patients were enrolled in the study,210 cases in normal healing group and 40 cases in delayed healing group.The proportions of combined diabetes mellius,open fracture and high-energy injury were higher in delayed healing group compared to normal healing group(χ^(2)=28.245,P=0.000;χ^(2)=24.881,P=0.00;χ^(2)=34.469,P=0.000).②The expression level of IncRNA PAX8-ASI in PVB was higher and the expression level of miRNA-1252-5p in PVB was lower in delayed healing group compared to normal healing group at postsur-gical week 2 and4(t=4.742,P=0.0;t=2.753,P=0.006;t=4.079,P=0.000;t=2.538,P=0.012).③
