摘要
目的探讨地塞米松联合阿奇霉素对大叶性肺炎患儿临床症状、肺功能及炎症因子水平的影响。方法选取2017年10月至2020年12月本院收治的98例大叶性肺炎患儿作为研究对象,按照随机数字表法分为对照组与实验组,每组49例。对照组给予阿奇霉素治疗,实验组在对照组基础上联合地塞米松治疗,比较两组肺部阴影消失时间、退热时间、咳嗽减轻时间及治疗前后肺功能指标[用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、用力呼出25%肺活量时呼气流量(FEF25%)]和血清炎症因子[C反应蛋白(CRP)、降钙素原(PCT)、红细胞沉降率(ESR)]水平。结果实验组咳肺部阴影消失时间、退热时间、咳嗽减轻时间均短于对照组,差异有统计学意义(P<0.05)。治疗后,两组FVC、FEV1、FEF25%水平均高于治疗前,且实验组高于对照组,差异有统计学意义(P<0.05)。治疗后,两组血清CRP、PCT、ESR水平均低于治疗前,且实验组低于对照组,差异有统计学意义(P<0.05)。结论地塞米松联合阿奇霉素可缓解儿童大叶性肺炎的临床症状,改善其肺功能,降低炎症因子水平,值得临床推广应用。
objective To investigate the effect of dexamethasone combined with azithromycin on clinical symptoms,pulmonary function and in-flammatory factors in children with lobar pneumonia.Methods 98 cases of children with lobar pneumonia were treated in our hospital from October 2017 to December 2020 were selected as the research subjects,and they were divided into the control group and the experimental group,with 49 cas-esin each group.The control group was treated with azithromycin,the experimental group was treated with dexamethasone on the basis of the control group.The pulmonary shadow disappearance time,antipyretic time,cough relief time,and pulmonary function factors(forced vital capacity[FVC],forced expiratory volume in one second[FEV1],forced expiratory flow at 25%of forced vital capacity[FEF25%])and serum inflammatory factors(C-reactire protein[CRP],procalcitonin[PC],erythrocyte sedimentation rate[ESR])levels before and after treatment were compared between the two groups.Results The pulmonary shadow disappearance time,antipyretic time,cough relief time in the experimental group were shorter than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of FVC、FEV1、FEF25%of the two groups were higher than those before treatment,and the experimental group was higher than the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of serum CRP、PCT、ESR of the two groups were lower than those before treatment,and the experi-mental group was lower than the control group,and the differences were statistically significant(P<0.05).Conclusion Dexamethasone combined with azithromycin can relieve the clinical symptoms of lobar pneumonia in children,improve pulmonary function,reduce the levels of inflammatory factors,which is worthy of clinical promotion and application.
