摘要
目的探讨甲状腺全切术在甲状腺癌外科治疗中的价值及安全性。方法选取2018年1月至2020年12月本院收治的54例甲状腺癌患者作为研究对象,按照随机数字表法分为对照组与研究组,每组27例。对照组使用甲状腺患侧腺叶切除术联合峡部切除术治疗,研究组使用甲状腺全切除术治疗,比较两组临床疗效、术后并发症发生情况、手术相关指标、术后复发率。结果研究组治疗总有效率高于对照组,差异有统计学意义(P<0.05);研究组手术时间、切口长度、术后住院时间、术后镇痛时间、术后下地活动时间均长于对照组,术中出血量多于对照组,差异有统计学意义(P<0.05);研究组术后并发症发生率高于对照组,术后复发率低于对照组,差异有统计学意义(P<0.05)。结论甲状腺全切除术治疗甲状腺癌患者效果显著,具有切除彻底、复发率低的优势,但会延迟术后恢复时间,增加术中出血量和手术时间,且并发症较多,临床治疗中应按照患者病情状况选择合理的手术方式。
Objective To investigate the value and safety of total thyroidectomy in the surgical treatment of thyroid carcinoma.Methods A total of 54 patients with thyroid cancer admitted to our hospital from January 2018 to December 2020 were selected as research subjects,and they were divided into control group and the study group according to the random number table method,with 27 cases in each group.The control group was treated with thyroid lobectomy combined with isthectomy,while the study group was treated with total thyroidectomy,the clinical efficacy,postoperative complications,operation-related indexes and postoperative recurrence rate were compared between the two groups.Results The total effective rate of the study group was higher than that of the control group,the difference was statistically significant(P<0.05).The operation time,incision length,postoperative hospital stay,postoperative analgesia time and postoperative ground movement time in the study group were longer than those in the control group,and the intraoperative blood loss was higher than that in the control group,the difference was statistically significant(P<0.05).The incidence of postoperative complications in the study group was higher than that in the control group,and the recurrence rate was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion Total thyroidectomy is effective in the treatment of patients with thyroid cancer,which has the advantages of complete resection and low recurrence rate,however,it will delay the postoperative recovery time,increase intraoperative blood loss and operation time,and lead to more complications,in clinical treatment,reasonable surgical methods should be selected according to the patients'conditions.
