摘要
目的探究经颅多普勒超声(TCD)联合血清Adropin、分泌型卷曲相关蛋白5(SFRP5)预测急性脑梗死(ACI)患者预后的价值。方法选取2020年1月至2022年7月本院收治的104例ACI患者为ACI组,另选取104例健康体检者为对照组。比较ACI组、对照组TCD参数平均血流速度(MV)、Adropin、收缩期峰值流速(PSV)、SFRP5、搏动指数(PI)水平;比较不同预后ACI患者一般资料及TCD参数MV、PSV、PI、血清Adropin、SFRP5水平;Logistic回归分析ACI患者预后的影响因素;受试者工作特征(ROC)曲线分析TCD参数MV、PSV、PI、血清Adropin、SFRP5预测ACI患者预后的价值。结果ACI组患者MV、PSV及血清Adropin、SFRP5水平低于对照组(P<0.05),PI高于对照组(P<0.05);预后不良组ACI患者MV、PSV及血清Adropin、SFRP5水平低于预后良好组(P<0.05),PI高于预后良好组(P<0.05);低水平PSV、Adropin、SFRP5、高水平PI均是ACI患者预后的危险因素(P<0.05);PSV、PI、血清Adropin、SFRP5预测ACI患者预后的曲线下面积(AUC)分别为0.697、0.828、0.855、0.865,且PI、血清Adropin、SFRP5联合预测ACI患者预后的AUC为0.978,其灵敏度为97.6%,特异性为85.5%。结论TCD参数PI与血清Adropin、SFRP5联合评估ACI患者预后的价值较高,其可能是临床评估ACI患者预后的潜在方法。
Objective To explore the prognostic value of transcranial Doppler ultrasound(TCD)combined with serum Adropin and secreted frizzled-related protein 5(SFRP5)in patients with acute cerebral infarction(ACI).Methods A total of 104 ACI patients admitted to our hospital from January,2020 to July,2022 were selected as ACI group,while 104 healthy people were selected as the control group.The mean blood flow velocity(MV),Adropin,peak systolic velocity(PSV),SFRP5 and pulsatility index(PI)of TCD parameters in ACI group and the control group were compared;The general data and TCD parameters MV,PSV,PI,serum Adropin,SFRP5 in patients with ACI with different prognosis were compared;Logistic regression analysis was used to analyze the prognostic factors of ACI patients,and the prognostic value of TCD parameters MV,PSV,PI,serum Adropin and SFRP5 in patients with ACI was evaluated by ROC curve.Results MV,PSV,and levels of Adropin and SFRP5 in ACI group were lower than those in the control group(P<0.05),while PI was higher than that in the control group(P<0.05);MV,PSV,levels of serum Adropin and SFRP5 in the poor prognosis group were lower than those in the good prognosis group(P<0.05),while PI was higher than that in the good prognosis group(P<0.05);Low level PSV,Adropin,SFRP5 and high level PI were all risk factors for prognosis of ACI patients(P<0.05);The area under the curve(AUC)predicted by PSV,PI,serum Adropin and SFRP5 for the prognosis of patients with ACI were 0.697,0.828,0.855 and 0.865,respectively.AUC of PI,serum Adropin and SFRP5 for predicting the prognosis of patients with ACI was 0.978,with sensitivity of 97.6%and specificity of 85.5%.Conclusion The combination of TCD parameter PI,serum Adropin and SFRP5 is of high value in evaluating the prognosis of patients with ACI,which could serve as a potential new method for clinical evaluation of the prognosis of patients with ACI.
