摘要
目的探究血液透析滤过(HDF)联合血液灌流(HP)治疗尿毒症的临床疗效。方法选取2019年12月至2020年11月于本院就诊的60例尿毒症患者作为研究对象,按照随机数字表法分为对照组与观察组,每组30例。对照组采用HDF治疗,观察组在对照组治疗基础上采用HP治疗。比较两组治疗前后14项尿毒症皮肤瘙痒量表(14-Item UP-Dial Scale)和大分子指标[促肾上腺皮质激素(ACTH)、甲状旁腺激素(PTH)白蛋白(ALB)、C反应蛋白(CRP)]。结果治疗前,两组尿毒症皮肤瘙痒量表评分差异无统计学意义;治疗4周后,两组尿毒症皮肤瘙痒量表评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗前,两组血清PTH、ACTH、ALB、CRP水平比较差异无统计学意义;治疗4周后,两组血清PTH、ACTH、ALB、CRP水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论HDF和HP联合治疗可改善尿毒症患者皮肤瘙痒情况,降低大分子水平。
Objective To explore the clinical effect of hemodiafiltration(HDF)combined with hemoperfusion(HP)in the treatment of uremia.Methods A total of 60 uremia patients admitted to our hospital from December 2019 to November 2020 were selected as research subjects,and they were divided into the control group and the observation group according to random number table method,with 30 cases in each group.The control group was treated with HDF,the observation group was treated with HP on the basis of control group.The 14 items of uremic pruritch scale and macromolecular indexes(adrenocorticotropic hormone[ACTH],parathyroid hormone[PTH],albumin[ALB],C-reactive protein[CRP])were compared between the two groups before and after treatment.Results Before treatment,there was no significant difference in the scores of uremia itch scale between the two groups;at the 4 weeks after treatment,the scores of uremic pruritus scale of the two groups were lower than those before treatment,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in serum PTH,ACTH,ALB and CRP levels between the two groups;at the 4 weeks after treatment,serum levels of PTH,ACTH,ALB and CRP of the two groups were lower than before treatment,and observation group was lower than control group,the differences were statistically significant(P<0.05).Conclusion HDF and HP combined therapy can improve the pruritus of uremia patients and reduce the level of macromolecules.
