摘要
目的探讨分体式髌骨爪结合张力带钢丝内固定治疗老年髌骨骨折患者的临床疗效。方法选取2015年2月至2020年2月本院收治的90例老年髌骨骨折患者作为研究对象,按照随机数字表法分为对照组与观察组,各45例。对照组采用张力带钢丝内固定治疗,观察组在对照组的基础上结合分体式髌骨爪固定治疗,比较两组膝关节功能恢复优良率、手术相关指标、治疗后不同时间段Lysholm评分、视觉模拟评分法(VAS)评分及相关并发症发生情况。结果观察组膝关节功能恢复优良率为97.78%,,明显高于对照组的82.22%,差异有统计学意义(P<0.05)。两组切口长度比较差异无统计学意义;观察组手术时间长于对照组,骨折愈合时间、住院时间均明显短于对照组,术后出血量明显少于对照组,差异有统计学意义(P<0.05)。治疗后3个月,两组Lysholm评分均明显高于治疗后2周,VAS评分均明显低于治疗后2周,且治疗后2周、3个月,观察组Lysholm评分均明显高于对照组,VAS评分均明显低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为6.67%,低于对照组的22.22%,差异有统计学意义(P<0.05)。结论采用分体式髌骨爪结合张力带钢丝内固定治疗老年人髌骨骨折效果较好,可提高膝关节功能恢复优良率,促进骨折愈合,减轻疼痛程度,改善膝关节功能。
Objective To investigate the clinical effect of split patellar claw combined with tension band steel wire internal fixation on elderly patients with patellar fracture.Methods 90 elderly patients with patellar fracture admitted to our hospital from February 2015 to February 2020 were selected as the study subjects,and they were divided into the control group and the observation group according to random number table method,with 45 cases in each group.The control group was treated with tension band steel wire internal fixation,and the observation group was treated with split patellar claw fixation on the basis of the control group,the excellent and good rate of knee functional recovery,surgery-related indexes,Lysholm score,visual analog score(VAS)at different time after treatment and the occurrence of related complications were compared between the two groups.Results The excellent and good rate of knee function recovery in the observation group was 97.78%,which was significantly higher than 82.22%in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in incision length between the two groups,the operation time of the observation group was longer than that of the control group,the fracture healing time and hospitalization time were significantly shorter than that of the control group,and the postoperative blood loss was significantly less than that of the control group,the difference was statistically significant(P<0.05).After 3 months of treatment,Lysholm scores of the two groups were significantly higher than those of 2 weeks after treatment,and VAS scores of the two groups were significantly lower than those of 2 weeks after treatment.At 2 weeks and 3 months after treatment,Lysholm score in the observation group was significantly higher than that in the control group,and VAS score was significantly lower than that in the control group,the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 6.67%,whi
