摘要
目的探讨特布他林联合清肺化痰汤治疗老年慢性阻塞性肺疾病(COPD)临床疗效。方法选取2019年1月至2021年1月本院收治的82例老年COPD患者作为研究对象,按照随机数字表法分为实验组与对照组,各41例。对照组采用特布他林治疗,实验组在对照组基础上加用清肺化痰汤治疗。比较两组临床疗效和治疗前后肺功能指标[第一秒用力呼气容积(FEV_(1))、第1秒用力呼气容积占用力肺活量比值(FEV_(1)/FVC)]、血清炎症因子[白细胞介素-6(IL-6)、白细胞介素-8(IL-8)]水平。结果实验组治疗总有效率为97.56%,高于对照组的78.05%,差异有统计学意义(P<0.05)。治疗后,两组咳痰、咳嗽、喘息积分均低于治疗前,且实验组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组FEV_(1)、FEV_(1)/FVC均高于治疗前,且实验组高于较对照组,差异有统计学意义(P<0.05)。治疗后,两组血清IL-6、IL-8水平均低于治疗前,且实验组低于对照组,差异有统计学意义(P<0.05)。结论特布他林联合清肺化痰汤治疗老年COPD疗效确切,可缓解炎症反应,改善肺功能和临床症状,值得临床推广应用。
Objective To investigate the clinical efficacy of terbutaline combined with Qingfei Huatan decoction in the treatment of chronic obstructive pulmonary disease(COPD)in the elderly.Methods A total of 82 elderly patients with COPD admitted to our hospital from January 2019 to January 2021 were selected as the research subjects,and they were divided into experimental group and control group according to random number table method,with 41 cases in each group.The control group was treated with terbutaline,the experimental group was treated with Qingfei Huatan decoction on the basis of the control group,the clinical efficacy,pulmonary function indexes(forced expiratory volume one second[FEV_(1)],forced expiratory volume in one second/forced vita 1 capacity[FEV_(1)/FVC])and serum inflammatory factors(interleukin-6[IL-6],interleukin-8[IL-8])before and after treatment were compared between the two groups.Results The total effective rate in the experimental group was 97.56%,which was higher than 78.05%in the control group,the difference was statistically significant(P<0.05).After treatment,the scores of expectoration,cough and wheezing of the two groups were lower than those before treatment,and the experimental group was lower than the control group,the difference was statistically significant(P<0.05).After treatment,the FEV_(1) and FEV_(1)/FVC of the two groups were higher than those before treatment,and the experimental group was higher than the control group,the differences were statistically significant(P<0.05).After treatment,serum IL-6 and IL-8 levels of the two groups were lower than those before treatment,and the experimental group was lower than the control group,the difference was statistically significant(P<0.05).Conclusion Terbutaline combined with Qingfei Huatan Decoction is effective in the treatment of elderly COPD,which can alleviate inflammation,improve lung function and clinical symptoms,and is worthy of clinical promotion and application.
