摘要
目的探究微创食管癌根治术对老年食管癌患者的治疗效果及对术后炎症及应激反应指标的影响分析。方法选取老年食管癌患者60例。按完全随机法将患者分为常规组和微创组,每组30例。常规组采用常规开胸术;微创组采用微创根治性食管癌根治术。结果2组性别、年龄、肿瘤直径和病理类型等一般资料比较差异不显著(P>0.05)。2组术前血清炎性因子水平差异不显著(P>0.05),手术后2 d 2组血清炎性因子水平均显著高于手术前,且微创组血清炎性因子水平显著高于常规组(P<0.05)。2组术前血清应激激素水平差异不显著(P>0.05),手术后2 d 2组血清应激激素水平均显著高于手术前,且微创组血清应激激素水平显著高于常规组(P<0.05)。微创组总有效率显著高于常规组(P<0.05)。结论微创食管癌根治术治疗老年食管癌疗效显著,可以降低炎症和应激反应,值得推广。
Objective To explore the therapeutic effect of minimally invasive radical resection of esophageal cancer on elderly patients with esophageal cancer and its influence on postoperative inflammation and stress response indicators.Methods 60 elderly patients with esophageal cancer were selected.The patients were randomly divided into conventional group and minimally invasive group,with 30 cases in each group.Conventional group received conventional thoracotomy;minimally invasive group was treated with minimally invasive radical esophagectomy.Results There were no significant differences in gender,age,tumor diameter and pathological type between the 2 groups(P>0.05).There was no significant difference in serum inflammatory factors between the 2 groups before operation(P>0.05).At 2 days after operation,the serum inflammatory factors in the 2 groups were significantly higher than those before operation,and the serum inflammatory factors in the minimally invasive group were significantly higher than those in the conventional group(P<0.05).At 2 days after operation,the serum stress hormone levels in the 2 groups were significantly higher than those before operation,and the serum stress hormone levels in the minimally invasive group were significantly higher than those in the conventional group(P<0.05).The total effective rate of minimally invasive group was significantly higher than that of the conventional group(P<0.05).Conclusion Minimally invasive radical resection of esophageal cancer has obvious therapeutic effect in the treatment of elderly esophageal cancer,which can reduce inflammation and stress response,and is worthy of promotion.
