神经电生理监测在面肌痉挛微血管减压中的应用价值

摘要 目的探讨神经电生理监测在面肌痉挛微血管减压中的应用价值。方法将2016-01—2018-08行面肌痉挛微血管减压术患者156例设为对照组,术中未用神经电生理监测,将2018-09—2020-05行面肌痉挛微血管减压术患者156例设为治疗组,术中应用神经电生理监测,回顾分析2组术后患者手术效果及对面、听神经的影响。结果治疗组即刻缓解138例(88.46%),术后1 a 148例(98.08%)治愈,对照组即刻缓解113例(72.43%),术后1 a 145例(97.44%)治愈,2组即刻缓解率比较差异有统计学意义(P=0.001),2组术后1 a总缓解率比较差异无统计学意义(P=0.318)。治疗组术后出现面瘫、耳鸣、听力障碍共7例(4.48%),对照组术后出现面瘫、耳鸣、听力障碍、吞咽障碍共26例(16.67%),2组比较差异有统计学意义(P<0.05)。结论面肌痉挛微血管减压术中应用神经电生理监测可以减少手术并发症,并对手术操作具有指导意义。 Objective To investigate the application value of neuroelectrophysiological monitoring in microvascular decompression of hemifacial spasm.Methods A total of 156 patients who underwent microvascular decompression for hemifacial spasm from January 2016 to August 2018 were set as the control group,and the nerve electrophysiological monitoring was not used during the operation.A total of 156 patients from September 2018 to May 2020 were selected as the treatment group,and nerve electrophysiological monitoring was applied during the operation.The postoperative effects of the two groups and the influence of the facial and auditory nerves were retrospectively analyzed.Results In the treatment group,138 cases(88.46%)were relieved immediately,and 148 cases(98.08%)were cured one year after the operation.In the control group,113 cases(72.43%)were immediately relieved,and 145 cases(97.44%)were cured one year after the operation.There was a significant difference in the remission rate(P=0.001),and there was no significant difference in the one-year total remission rate between the two groups(P=0.318).There were 7 cases(4.48%)of facial paralysis,tinnitus and hearing impairment in the treatment group,and 26 cases(16.67%)of facial paralysis,tinnitus,hearing impairment and dysphagia in the control group.There was a significant difference between the two groups(P<0.05).Conclusion The application of neurophysiological monitoring in microvascular decompression for hemifacial spasm can reduce surgical complications and has guiding significance for surgical operation.
