血清GSH-PX MDA及SOD预测短暂性脑缺血发作患者不良预后的价值 被引量:2

摘要 目的分析血清丙二醛(MDA)、超氧化物歧化酶(SOD)及谷胱甘肽过氧化物酶(GSH-PX)水平与短暂性脑缺血发作(TIA)患者不良预后的关系。方法选取2021-02—2022-02济南市人民医院治疗的110例TIA患者为研究对象,检测血清MDA、SOD及GSH-PX水平。所有患者均随访1 a,根据其预后情况分为预后良好组与预后不良组。单因素及Logistic回归分析影响TIA患者预后的危险因素,绘制受试者工作特征(ROC)曲线分析血清MDA、SOD及GSH-PX水平对TIA患者预后不良的预测价值。结果预后不良组患者高血压、糖尿病、症状持续时间≥60 min占比及血清MDA水平均明显高于预后良好组,血清SOD及GSH-PX水平均明显低于预后良好组(P<0.05);Logistic分析显示,高血压、糖尿病、症状持续时间及血清MDA、SOD、GSH-PX是影响TIA患者预后的危险因素(P<0.05);ROC曲线分析显示,血清MDA、SOD、GSH-PX水平预测TIA患者预后不良的曲线下面积(AUC)分别为0.846、0.793、0.827,三者联合预测TIA患者预后不良的AUC为0.921,明显大于三者单独应用(P<0.05)。结论血清MDA水平异常升高、SOD及GSH-PX水平异常下降是TIA患者预后不良的危险因素,三者联合预测TIA患者预后不良的价值较高。 Objective To analyze the relationship between the levels of serum malondialdehyde(MDA),superoxide dismutase(SOD)and glutathione peroxidase(GSH-PX)and the adverse prognosis of patients with transient ischemic attack(TIA).Methods Totally 110 TIA patients admitted to Ji’nan People’s Hospital from December 2021 to December 2022 were selected as the study subjects,and the serum MDA,SOD,and GSH-PX levels were measured.All patients were followed up for 1 year and were divided into good prognosis group and poor prognosis group based on their prognosis.Single factor and Logistic regression analysis were used to identify the risk factors that affect the prognosis of TIA patients,and receiver operating characteristic(ROC)curves were drawn to analyze the predictive value of serum MDA,SOD,and GSH-PX levels for poor prognosis in TIA patients.Results The proportion of hypertension,diabetes,symptom duration≥60 min and the content of serum MDA in the poor prognosis group were significantly higher than those in the good prognosis group,while the content of se⁃rum SOD and GSH-PX were significantly lower than those in the good prognosis group(P<0.05).Logistic analysis showed that hypertension,diabetes,duration of symptoms,serum MDA,SOD and GSH-PX levels were risk factors affecting the prognosis of TIA patients(P<0.05).ROC curve analysis showed that the area under curve(AUC)of serum MDA,SOD,and GSH-PX levels predicting poor prognosis in TIA patients was 0.846,0.793,and 0.827,re⁃spectively.The AUC of combining the three predicted poor prognosis in TIA patients was 0.921,which was signifi⁃cantly greater than that predicted by the three alone(P<0.05).Conclusion Abnormal increase in serum MDA content and abnormal decrease in serum SOD and GSH-PX are risk factors for poor prognosis in TIA patients,and the combined detection of the three has high value in predicting poor prognosis in TIA patients.
