摘要
目的探索帕金森病(PD)患者血浆外泌体中微小RNA-331-5p(miR-331-5p)的表达及临床意义。方法选取2020-01—2022-12于新疆医科大学第一附属医院诊治的PD患者78例作为研究对象(PD组),另纳入同期无任何基础疾病的健康志愿者36例为对照组。检测2组患者血浆外泌体中miR-331-5p水平、血清中炎症及氧化应激指标水平,评估2组患者认知功能及精神行为症状,分析miR-331-5p与炎症、氧化应激、认知功能及精神行为症状的关系,探讨PD发病的影响因素,计算miR-331-5p对PD发病的诊断价值。结果PD组患者血浆外泌体miR-331-5p水平显著高于对照组(P<0.05)。PD组患者神经精神调查问卷(NPI)评分、PD评定量表运动检查(UPDRS)评分显著高于对照组,生活活动能力量表(ADL)评分、蒙特利尔认知评估量表(MoCA)评分及简易精神状态检查表(MMSE)评分明显低于对照组(P<0.05)。PD组患者血清髓过氧化物酶(MPO)、白细胞介素-1β(IL-1β)、超氧化物歧化酶(SOD)、白细胞介素-6(IL-6)、丙二醛(MDA)、肿瘤坏死因子-α(TNF-α)、晚期氧化蛋白产物(AOPP)水平显著高于对照组(P<0.05)。PD组患者血浆外泌体miR-331-5p与IL-1β、IL-6、TNF-α、SOD、MDA、AOPP呈正相关(P<0.05)。PD组患者血浆外泌体miR-331-5p与NPI评分、UPDRS评分呈正相关,与MoCA评分、MMSE评分、ADL评分呈负相关(P<0.05)。Logistic多因素回归分析显示miR-331-5p(OR=2.024)、MDA(OR=1.925)、UPDRS评分(OR=2.447)、MoCA评分(OR=0.544)均是PD发病的影响因素(P<0.05)。miR-331-5p诊断PD的ROC曲线下面积(AUC)为0.809(95%CI:0.715~0.903),截断值为0.833(2-ΔΔCt),灵敏度为75.6%,特异度为77.8%。结论血浆外泌体miR-331-5p与PD患者炎症、氧化应激反应、认知功能及精神行为症状密切相关,且对PD的发生具有良好的诊断价值,可为早期诊治提供新方向。
Objective To the expression and clinical significance plasma exosomal micro-RNA-331-5p(miR-331-5p)in patients with Parkinson’s disease(PD).Methods A total of 78 PD patients treated in the First Aftilicoted Hospital of Xinjing Medical University from January 2020 to December 2022 were selected as the study object(PD group),and 36 volunteers without any history of neurological or psychiatric diseases were included as the control group.The levels of miR-331-5p in plasma exosomes and the levels of inflammation and oxidative stress indicators in serum of the two groups were detected,the cognitive function and psychobehavioral symp⁃toms of the two groups were evaluated,and the relationship between miR-331-5p and inflammation,oxidative stress,cognitive function and psychobehavioral symptoms was analyzed,and the influencing factors for PD inci⁃dence were explored.The diagnose value of miR-331-5p for PD was calculated.Results Plasma exosome miR-331-5p level in PD group was significantly higher than that in control group(P<0.05).PD patients nervous spirit questionnaire(NPI)score,PD rating scale movement examination(UPDRS)mini significantly higher than the control group,the Montreal cognitive assessment scale(MoCA)score,mini mental state check table(MMSE)score,actiutiy of living(ADL)scale score significantly lower than the control group(all P<0.05).PD patients’serum myeloperoxidase(MPO),interleukin 1β(IL-1β),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),superoxide dismutase(SOD),malondialdehyde(MDA),advanced oxidation protein products(AOPP)were signifi⁃cantly higher than control group(P<0.05).Plasma exosome miR-331-5p level in PD group and IL-1β,IL-6,TNF-α,SOD,MDA,AOPP had significantly positive correlation(P<0.05).PD patients Plasma exosome miR-331-5P and NPI score,UPDRS score had positive correlation,and MoCA score,MMSE scores and ADL scores show negative correlation(P<0.05).Logistic multifactor regression analysis showed that miR-331-5p(OR=2.024),MDA(OR=1.925),UPDRS score(OR=2.447),the influence fact
