摘要
目的:探讨超声造影定量参数联合血清circCELSR1检测在早期卵巢癌中的诊断价值及其与淋巴结转移的关系。方法:收集2020年5月-2022年10月本院收治的96例卵巢癌患者临床资料为卵巢癌组,根据是否发生淋巴结转移分为淋巴结转移组44例和无淋巴结转移组52例;选取同期卵巢良性肿瘤患者94例作为良性肿瘤组。采用彩色超声诊断仪对两组患者进行超声造影检查,获取始增时间、达峰时间、灌注时间、增强强度,采用实时荧光定量PCR法测定血清circCELSR1水平;采用多因素logistic回归分析检验超声造影定量参数及血清circCELSR1水平与卵巢癌的关系;绘制受试者工作特征(ROC)曲线检验超声造影定量参数及血清circCELSR1水平对卵巢癌的诊断价值。结果:卵巢癌组始增时间、达峰时间低于良性肿瘤组,增强强度及血清circCELSR1水平高于良性肿瘤组,且均与卵巢癌发生有关(均P<0.05)。始增时间、达峰时间、增强强度、血清circCELSR1水平及4项联合诊断卵巢癌的曲线下面积(AUC)分别为0.825、0.854、0.846、0.762、0.970。卵巢癌组淋巴结转移患者的始增时间、达峰时间低于无淋巴结转移患者,增强强度及血清circCELSR1水平高于无淋巴结转移患者(均P<0.05)。结论:采用超声造影定量参数联合血清circCELSR1水平检测可提高卵巢癌诊断效能,且各指标水平与淋巴结转移有关,可为临床诊疗提供依据。
Objective:To investigate the value of contrast-enhanced ultrasound quantitative parameters combined the se-rum circCELSR1 detection for early diagnosing ovarian cancer of patients,and to study its correlation with the lym-phatic metastasis of ovarian cancer.Methods:The clinical data of 96 patients with ovarian cancer(in study group)from May 2020 to October 2022 were collected,including 44 patients with the lymphatic metastasis in group A and 52 patients without the lyrmphatic metastasis in group B according to whether the lymphatic metastasis occurred or not.94 patients with ovarian benign tumors were selected in control group.The patients in these groups were examined by col-or ultrasound diagnostic apparatus to obtain the starting time,the peak time,the perfusion time,and the enhancement intensity of the contrast medium.The serum circCELSR1 level of the patients in these groups was measured by real-time fluorescent quantitative PCR.Multivariate Logistic regression analysis was used to analyze the correlation between the quantitative parameters of contrast enhanced ultrasound and the serum circCELSR1 level of the patients and their ovarian cancer occurrence.Receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of the quantitative parameters of contrast enhanced ultrasound and the serum circCELSR1 level of the patients for their o-varian cancer.Results:The starting time and the peak time of the contrast medium of the patients in the study group were significantly lower than those of the patients in the control group,and the enhancement intensity of the contrast medium and the serum circCELSRI level of the patients in the study group were significantly higher,and both of which were associated with the ovarian cancer occurrence(all P<0.05).The area under the curve(AUC)of the starting time,the pcak time,the enhancement intensity,and the scrum cirCELSR1 level,and the combined of the contrast medium starting time,peak time,enhancerment intensity,and the serum circCELSR1 level of the pat
