摘要
目的探讨超声引导下射频消融(RFA)治疗3~5 cm肝细胞癌(HCC)的临床效果。方法回顾性收集2017年1月至2021年12月期间温州医科大学附属第五医院46例超声引导下行RFA治疗的3~5 cm肝细胞癌患者临床资料,其中21例患者在消融术后即刻行超声造影,评估是否需要补充消融,设为研究组;另外25例患者在消融术后,采用常规超声检查来评估是否需要补充消融,设为对照组。对比分析两组患者的技术成功率、消融比率、术后复发率、无进展生存期(PFS)、总生存期(OS)及并发症。结果两组患者消融治疗的技术成功率均为100.0%。研究组消融次数更多,消融比率高于对照组(P<0.05)。研究组消融术后复发率明显低于对照组(P<0.05),且OS明显高于对照组(P=0.041)。结论超声引导下RFA治疗3~5 cm的HCC安全、可靠,消融术后即刻行超声造影,可及时评估消融的彻底性,降低肝癌患者消融术后复发率,延长患者的OS。
Objective To compare the clinical outcomes of radiofrequency ablation(RFA)guided by ultrasound for the treatment of patients with hepatocellular carcinoma(HCC)of 3-5 cm in diameter.Methods The clinical data of 46 patients,with HCC of 3-5 cm in diameter,who treated with RFA under ultrasound guidance in the Fifth Affiliated Hospital of Wenzhou Medical University from Jan.2017 to Dec.2021 were retrospectively analyzed.Among them,21 patients underwent contrast-enhanced ultrasound immediately(research group)and the other 25 patients received routine ultrasound examination(control group)after RFA.The technical success rate,frequency of ablation,ablation rate,postoperative recurrence rate,PFS,OS and complications of the two groups were compared and analyzed.Results The technical success rate of RFA treatment in both groups was 100.0%.The frequency and ablation rate in research group was significantly higher than that in the control group(P<0.05).The recurrence rate of the research group was significantly lower than that of the control group(P<0.05).The survival time in the resarch group was also significantly higher than that in the control group(P=0.041).Conclusion Ultrasound-guided RFA is safe and reliable treatment for HCC of 3-5 cm in the diameter.Performing contrast-enhanced ultrasound immediately after RFA can timely evaluate the completeness of ablation,reduce the recurrence rate of liver cancer after RFA,and prolong the overall survival of patients.
