摘要
目的:分析比较进行经桡动脉穿刺插管心、脑血管造影术时运用四种穿刺针进行穿刺的效果。方法:按照穿刺术中、术后观察的方式开展研究,选入2021年1月至2022年6月在柳州市中医医院进行桡动脉穿刺插管心、脑血管造影术的患者400例作为研究对象,采用双盲法均分为四组,每组各100例。其中观察组(T组)采用BD Insyte-W^(TM)20GA静脉留置针穿刺,对照一组(M组)采用日本泰尔茂5F桡动脉鞘配套穿刺针穿刺,对照二组(N组)采用Cordis5F桡动脉鞘配套穿刺针穿刺,对照三组(F组)采用一次性经皮穿刺针穿刺。比较四组的一次性穿刺成功率、穿刺过程中的渗血率、渗血量、疼痛程度及术后并发症的发生率。结果:(1)T组的一次性穿刺成功率高于N组及F组(P<0.05);T组、M组的一次性穿刺成功率相比无统计学差异(P>0.05);N组、F组的一次性穿刺成功率相比无统计学差异(P>0.05)。(2)T组、M组穿刺过程中的渗血率及渗血量相比无统计学差异(P>0.05);T组、M组穿刺过程中的渗血率和渗血量均低于N组、F组(P<0.05)。(3)T组、M组的术后并发症发生率相比无统计学差异(P>0.05);T组、M组的术后并发症发生率均低于N组、F组(P<0.05)。(4)T组穿刺过程中的视觉模拟评分法(VAS)评分均低于其他三组(P<0.0)。结论:BD Insyte-W^(TM)20GA静脉留置针可以运用于经桡动脉穿刺插管心、脑血管造影术中,具有一次性穿刺成功率高、疼痛轻、渗血少、并发症发生率低等优势。
Objective:To analyze and compare the effect of four kinds of puncture needles in cardiac and cerebral angiography with radial artery puncture.Methods:A total of 400 patients who underwent radial artery puncture intubation cardiography and cerebrovascular angiography in Liuzhou Hospital of Traditional Chinese Medicine from January 2021 to June 2022 were selected as the study objects and were divided into four groups with 100 cases in each group by double-blind method.Observation group(group T)was punctured with BD Insyte-W^(TM)20GA intravenous indwelling needle,control group(group M)was punctured with Japan Telmaw 5F radial sheath matching puncture needle,control group(group N)was punctured with Cordis5F radial sheath matching puncture needle,and control group(group F)was punctured with one-time percutaneous puncture needle.The success rate of one-time puncture,the rate of blood infiltration during puncture,the amount of blood infiltration,the degree of pain and the incidence of postoperative complications were compared among the four groups.Results:(1)The success rate of one-time puncture in T group was higher than that in N and F groups(P<0.05);There was no significant difference in the success rate of one-time puncture between T group and M group(P>0.05).There was no significant difference in the success rate of one-time puncture between group N and group F(P>0.05).(2)There were no statistically significant differences in blood infiltration rate and volume between T group and M group(P>0.05).The blood infiltration rate and volume in T and M groups were lower than those in N and F groups(P<0.05).(3)There was no significant difference in the incidence of postoperative complications between group T and group M(P>0.05).The incidence of postoperative complications in groups T and M was lower than that in groups N and F(P<0.05).(4)The visual analogue scale(VAS)scores in group T were lower than those in the other three groups(P<0.0).Conclusion:BD Insyte-W^(TM)20GA intravenous indentation needle can be used in cardiac
